**OUTREACH WORKER (Harm Reduction/HRO)***

**Division**: Community and Justice/Harm Reduction
**Work Status**: Part Time
**Location**: Durham Region (all offices as required)

**About the Role**:
**You will be working on things like.....**

**Service User/Customer Service**
- Provide program support services with individuals dealing with substance use
- Actively seek out service users in the community and establish trust through programming and education relating to harm reduction, safety, and health practices
- Facilitate exchanges by contributing to the nonjudgmental access to sterile equipment/supplies and safe disposal of used items as requested
- Engage any opportunities to support service user goals around any harm reduction measures relating to their drug use or health in general
- Provide referral where necessary and requested to the appropriate resources for service users
- Assist service users to arrange/schedule appointments and accompany them when requested or as needed
- Assist and provide service users with practical supports as available
- Assist clients with completing forms as required to access services
- Develop and facilitate groups as needed

**Case Management/Documentation**
- Follow program best practices guidelines to ensure quality of service
- Keep accurate statistical data related to program and ensure all forms and reports are completed as required
- Maintain filing systems, basic databases and records
- Maintain up-to-date case notes, progress reports and service user files with clear rationale for methods and intervention used, and statistical documentation in accordance with the program requirements
- Complete accurate statistical and narrative reports
- Monitor and maintain financial tracking where required
- Meet all required deadlines
- Contribute to the ongoing development of new systems and record keeping methods to ensure funding requirements are met and all documentation is complete, accurate and accountable

**Community Liaison**
- Assist with program promotion
- Work with other community services to reduce stigma and decrease barriers to accessing those services of importance to our service users
- Collaborate with other community services/partners on various service user based programming whenever possible
- Provide education and information about Hepatitis C, HIV/AIDS and STBBI’S (sexually transmitted and blood born infections) and promote/encourage testing and treatment for same
- Facilitate Naloxone training and distribution
- Contribute to the recruitment, training and mentoring of peer workers
- Arrange for informational workshops to be held at various times
- Attend any workshops, seminars or meetings as directed by the funder or deemed appropriate by the Harm Reduction Program Coordinator
- Provide information to staff, organizations and/or service users about agency programs and services.
- Create and maintain promotional information as needed

**If you have the following we would love to hear from you**
- Minimum of College diploma in Addictions and Mental Health
- Previous experience in addictions counselling, social service work and/or health care is required
- Non-judgmental and flexible
- Demonstrate strong interview and counselling skills
- Able to write simple correspondence, including memos, letters, etc.
- Basic mathematical skills
- Adjusts and is flexible to meet changing work needs and demands
- Superior telephone manners and strong interpersonal skills
- Strong written and verbal communication skills
- Must have own vehicle with minimum of $2,000,000 liability insurance
- Must have Hepatitis B vaccinations

Status: Part Time

Hours**:10 hours per week, Monday to Friday (schedule to be determined)**

Location: Durham Region

Reports to: Coordinator, Harm Reduction Services

Travel: Yes, as required (mileage reimbursement)

**Salary**: $22.81 per hour to start
