**Job Summary**:
Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Central regions’ Behaviour Support Action Plan. A key component of the plan is the expansion of the Behaviour Support Outreach Team (‘BSOT’), consisting of interdisciplinary community outreach clinicians, nurses and personal support workers who have behavioural support expertise. The Clinician leader will travel and work in the long term care and community sectors to help with managing responsive behaviours, capacity building, and data collection. The Clinician Leader will complete client observations and assessments, recommend appropriate interventions, model strategies and assist with transitions across sectors.

**Responsibilities include but are not limited to**:

- Provides clinical operational leadership support to the BSO program focusing on optimizing care delivery on specific cases and on the BSO active patient population as a whole
- Participates in relevant data collection, education and research activities, and performs duties consistent with the job classification, as required
- Provides comprehensive clinical assessments with a focus on identifying factors contributing to responsive behaviors utilizing a bio-psychosocial framework
- Collaborates with clients, care partners, and the client’s circle of care to develop and disseminate individualized behavior management plans in the community and long-term care sectors
- Collaborates with the VBM care team, client, care partners, and the client’s circle of care in the provision of VBM services to optimize care co-ordination and provides advice, recommendations and solutions on the most complex cases
- Collaboratively develops and plans assignments with PSWs, community agencies and Long-Term Care Teams
- Escalates issues to the Clinical Manager as appropriate
- Monitors and reports weekly on BSO Wait times, number of referral, and other data requested by the Clinical Manager
- Identifies opportunities and takes leadership role to establish linkages with partnering organizations to promote and advance the BSO program and model
- Coordinates and ensures the effective use of resources to meet client needs
- Participates in relevant research activities and performs duties consistent with the job classification, as required
- Reviews complex referrals in collaboration with BSO coordinating office
- Work side by side with Toronto Home and Community Care Support Services (HCCSS) teams and other community providers to provide behavioural support expertise, coaching and building capacity through partnerships
- Collaborate and liaise with hospital, community groups, crisis teams, and other community teams to develop an integrated pathway for clients
- Facilitate transitional support from community into acute care or the long-term care sector
- Perform teaching responsibilities, participate in relevant education and research activities, and perform duties consistent with the job classification, as required
- Perform departmental responsibilities to contribute to the efficient and effective provision of service
- Ensure the protection of client rights and acting in an advocacy role for the resident/clients

**Qualifications include but are not limited to**:

- Completion of a Master’s degree program in a healthcare profession
- Current registration/licensure in good standing as required by a professional body
- Minimum three (3) years related experience in geriatrics, community-based seniors care and behavioural management
- Experience/familiarity with Resident Assessment Instrument—Home Care© (RAI-HC) and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours
- Demonstrated expertise and knowledge of working with individuals with various types of dementia, delirium, and those with acute behavioural change
- Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioural support management approaches
- Demonstrated excellent clinical-reasoning, critical thinking and problem-solving skills
- Training in or knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. U-First, Montessori Approach
- Effective assessment, intervention, client centered and collaborative care planning skills
- Experience providing caregiver education, support and counselling relating to caring for a person with responsive behaviours
- Demonstrated experience working collaboratively with primary care and specialists in the care of older adults with responsive behaviours
