1. Provide counseling to clients:

- Conduct psychosocial assessments of clients and develop appropriate intervention plans and reports;
- In collaboration with clients, their families and clinical team, assess treatment needs and develop individual care and service plans;
- Provide clients with outcome oriented, time-limited counselling;
- Communicate necessary and relevant information to clients, agencies, staff, interprofessional team members, and family where appropriate;
- Coordinate and monitor care plan delivery;
- Administer outcome tools where appropriate;
- Participate as needed in case conferences;
- Strong interpersonal skills to function as a productive member of an inter-professional team with an understanding of relevant roles required;
- Skills in alternative communication styles an asset (for persons with fluency and/or articulation disorders, hard of hearing, mild aphasias, executive dysfunction, or impaired comprehension);
- Experience working with persons with impaired insight and judgement required;
- Facilitate therapeutic groups based on identified needs, and report on results of the group as appropriate.

2. Provide education to staff as part of treatment implementation:

- Liaise with management, clinical team and employees to ensure that recommendations for residential clients are understood and can be implemented;
- Develop Emergency Support Plans in collaboration with the clinical team, and implement with residential staff.

3. Act as a resource regarding community services for clients, staff, and partners:

- Comprehensive knowledge of regional community resources in Hamilton and Niagara areas;
- Aid client with navigating external service providers/refer as required;
- Keep abreast of changes in legislation that have the potential to impact clients;
- Balance client needs and choices with available resources, ensuring clients’ values and preferences are respected;
- Provide consultation to inter-professional team members and care coordination to individuals;
- Inform the agency of service trends that may influence program development;
- Demonstrates professional and ethical business practices;
- Adopt a collaborative interdisciplinary approach with internal and external team members.

4. Indirect/Administrative Services:

- Maintain accurate funding and quality statistics as required by the agency;
- Prepare and maintain client records for individuals served based on documentation protocols and regulatory authority;
- Travel as needed to residential sites across the Hamilton and Niagara areas;
**Minimum Qualifications**:

- A bachelor's degree in social services, mental health services, social work, or equivalent education;
- Three years’ work experience in social or health services;
- Additional training in the following areas an asset:

- Trauma-informed counselling practices (e.g. Acceptance and Commitment Therapy)
- Addictions, harm reduction, and/or suicide intervention
- Excellent relationship building skills
- Excellent written and verbal communication skills
- Proficient in computer software programs such as word, excel and power point
- Access to a reliable vehicle and a valid driver’s licence is required
- Valid First Aid/CPR certificate is an asset
