**About the Opportunity**:
Reporting to the Health Services Manager, Primary Health Care, the Interdisciplinary Clinical Leader provides leadership, innovation and support to the clinical team in the provision of interdisciplinary, collaborative, person-centered care to patients with a focus on functional health. The Interdisciplinary Clinical Leader works closely with patients and families, team members (physicians, psychologists, occupational therapists, physiotherapists, social workers, nurses, office staff) family physicians, community organizations, primary, secondary and tertiary health services, to promote person-centered collaborative care within in the Chronic Pain Management portfolio.

The incumbent develops and fosters a healthy interdisciplinary team processes that develop a person and family centered continuum of care focused on functional health goals, coordinating community based services of the Chronic Pain Management program, program planning and evaluation - ensuring practice is interdisciplinary and evidence based, participating in the recruitment, selection and orientation of new team members, supporting a quality approach to care delivery, coordinating and providing clinical care to patients. Duties include triaging of referrals, contributing intakes, care coordination and case management, patient navigation of care pathways, patient education and coverage for team members.

**About you**:

- Bachelor of Science or equivalent in Occupational Therapy.
- Licensure with the provincial regulatory body, the College of Occupational Therapists of Nova Scotia, is mandatory.
- 5 years' related experience or Master's plus 3 years related experience required. This includes two years of required experience working with people who have chronic pain, in a Primary Health Care setting/community based team/community setting and experience with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles; experience working with people with chronic conditions in an exercise program or setting.
- Experience in partnership development, stakeholder collaboration, and community development
- Understanding of population health, health promotion strategies, and behavior change principles
- In-depth knowledge of NSH, primary health care and community based services.
- Demonstrates person-centered / family-focused, diversity relevant approaches to care
- Well developed interpersonal, teamwork, oral and written communication skills
- Demonstrates flexibility, adaptability and willingness to change
- Demonstrates innovation in program planning, implementation and evaluation, and quality improvement, planning skills

**About You cont.**:

- Licensed with the respective Nova Scotia regulatory college
- Excellent organizational and time management skills
- Experience working with people who have complex chronic health conditions and pain
- Experience with leadership of multidisciplinary clinical teams
- Excellent interpersonal and communication skills
- Ability to work as a team and to prioritize while under pressure for deadlines
- Excellent research and program evaluation skills
- Proficient in microsoft office products
- Familiarity with primary care electronic medical record preferred
- Knowledge and understanding of Demonstrated commitment to diversity, equity and inclusion
- Competencies in other languages an asset, French preferred

**Hours**:

- Permanent part-time; 60% position; 45 hours bi-weekly
- Shifts include days, and may include evenings and weekends.
- Travel to various sites may be required.

**Compensation and Incentives**:
$38.06 - $46.35 Hourly
