**Description of Duties and Responsibilities**
The Health Care Aide (HCA) reports directly to the LPN, adheres to ethical standards in all work activities, and provides services within a harm reduction model. The HCA will be responsible for providing daily assistance as needed in a harm reduction and palliative care model which will include medication administration, wound care, enhancing quality of life, managing symptoms, end of life care and wishes of the clients loved ones.
The LPN understands and respects clients wish to die in comfort, with privacy and dignity at the last phase of one’s life and has a good understanding of the different stages clients and their families will experience throughout out the illness and ending stages.
They will be responsible for client ADLs, personal care, and hygiene and will be expected to function well in a team environment while following the policies and procedures of the George Spady Society. Working closely with the Program Manager and the LPN, the HCA will help ensure all clients’ medical and psychosocial needs are being met. In that context, the HCA duties will include:
- Adhere to the George Spady Society Code of Ethics
- Relief of pain and other physical symptoms such as fatigue, loss of appetite and difficulty breathing
- Emotional support, listening and answering questions from clients or their loved ones
- Ensure cultural, religious and spiritual needs are fulfilled
- Ensure that clients family wishes are fulfilled when possible
- Work together with program team
- Participate in morning and afternoon shift change meetings
- Interact with clients in a nonjudgmental way, using de-escalation and rapport building techniques
- Maintain case notes and update the client files
- Completes daily rounds to ensure safety
- Update and maintain information on referral sources using the FYI database
- Attend staff meetings as required
- Participate in ongoing learning and training opportunities as identified on yearly performance review
- Any other tasks assigned by the LPN
**Position Qualifications**
- Qualified HCA (must provide current Directory number)
- Demonstrates understanding of the addictions field
- Experience working with people with complex life circumstances associated with substance-related disorders, mental health issues, mobility issues and homelessness
- Basic understanding of harm reduction principles based on dignity, hope and healing as individuals move forward towards improved health and well-being
- An open, accepting and non-judgmental attitude with an ability to build rapport with clients and staff
- Excellent computer, typing and interpersonal skills
- Current Police with Vulnerable Sector Check
- Current First AID, CPR, AED certification
**Hours of work**: 8 hour shifts on a 4 on, 4 off rotation
