**2 YEAR COLLEGE DIPLOMA REQUIRED, DSW or other Human Services Related Diploma**

- **Responsibilities**:_
- **S**upport adults who have intellectual and developmental disabilities in accordance with their support plan
- **T**ake a lead role in developing support plan
- **L**iaise with other professionals, family, physicians and community
- **B**e part of leading a team
- **C**oordinate support team for individuals on caseload

As a Not-for-profit organization Community Living Haldimand has been supporting individuals with intellectual and developmental disabilities since 1958. We are located in Haldimand County, with this position currently located in Dunnville, ON.

Schedule:

- 8 hour shift
- Day shift
- Evening shift
- Monday to Friday
- Weekend availability

**Education**:

- DCS / DEC (required)

**Experience**:

- social services: 3 years (preferred)
