The Young Parent Program provides a comprehensive range of supports for pregnant and parenting youth and their children. Day Treatment, Residence and Outreach programs serve to strengthen the lives of young parents and their children by offering support, encouragement and hope in a safe welcoming environment.

Ujima Project at Catholic Family Services, St. Martin’s Manor - Young Parent Program is an initiative of the Ontario Association of Young Parents Agencies that is comprised of over twenty agencies throughout Ontario that provide programs and services to vulnerable adolescent parents and their children. This initiative will plan and implement 9 Early Childhood Development Hubs throughout Ontario. St. Martin’s Manor will be one of these 9 hubs.

The Family Navigator will be a key contact for young parents and their children seeking resources to help them live independently in the community or explore kinship or co-parenting options. Young parents and their children will be partnered with the Family Navigator who will engage in a collaborative approach to determine the range of their service needs and readiness to access treatment/care. The Family Navigator will facilitate access to timely care, utilizing detailed and extensive knowledge of mental health, infant and early childhood attachment and be a “bridge” between the family and services for young parents offered by community agencies.

This position requires a highly motivated and self-directed professional with a degree in health promotion, child development, child and youth care or psychology or a relevant field or an acceptable level of education, skills and experience and 3 years’ experience working within the community-based child and youth mental health or the young parent and infant sectors, preferably with youth families and their children.

To be successful in this role, you have working knowledge of child development and infant/maternal health issues, new parent and family engagement framework, and are able to effectively engage hard-to-reach young parents. You are passionate about working with marginalized, racialized and/or LGTBQ2S youth, and understand infant and early childhood mental health, housing, community support services, youth mental health and addictions services across the region.

You excel at understanding the barriers and challenges faced by youth parents and their families in culturally diverse, newcomer, and/or low-income communities. You shine in the area of youth/family engagement and rapport building skills with an understanding of youth-focused, client-centered care that is offered in a non-judgmental, strength-based, trauma-informed environment. You are able to establish effective working relationships with relevant public, private sector service providers and other community agencies, healthcare services and resources to assist youth parents and their families with accessing housing, healthcare, infant mental health, trauma, attachment, mental health/addictions services and to support successful introductions to services.

You have program planning, coordination and evaluation skills and the ability to set goals and get results within a pre-determined timeline. Your ability to multi-task and work independently and your flexibility in accepting work assignments is complemented by your effectiveness at time management, decision-making and organizational skills. You are a good team member and supportive of interdisciplinary practice with excellent interpersonal and communication skills and an ability to work effectively with colleagues and clients. You have demonstrated knowledge of Microsoft Office Suite.

Must be able to successfully complete a medical and police clearance with the vulnerable sector screen. You must have current Standard First Aid/CPR certification. You are required to travel off-site to community locations using your own vehicle.
