**Warden Woods Community Center**

Warden Woods has been working collaboratively to meet the needs and interests of local community for over 50 years in Southwest Scarborough. We are a multi-service, charitable organization delivering programs and services across 6+ local sites with a workforce of 80 employees and 120 volunteers, in partnership with over 50 other groups. We currently operate a 49-bed 24/7 respite program, daycare, early years drop-in, children’s camps and afterschool programs, youth drop-in’s, drop-in and food security programs, meals on wheels and transportation support, 24/7 assisted living, home help, seniors’ recreation and congregate dining, intergenerational community events, tax clinics, adult recreation and health + wellness programs, advocacy and civic engagement projects, and other opportunities year-round for the benefit of local people and the Southwest Scarborough community as a whole.

**Position Summary**

Reporting to the Manager Seniors & Assisted Living, the Caregiver Support Coordinator is responsible for ensuring that caregivers in Southwest Scarborough are supported in their role of caring for aging people or people with disabilities. The Caregiver Support Coordinator will work directly with caregivers 1-on-1 and in group settings, conduct outreach, plan/implement events and activities for caregivers and coordinate with program partners to ensure program success.

**Responsibilities**:

- Plan and implement outreach strategies to grow program participation within the Southwest Scarborough community
- Design, plan and implement relevant program activities that address the needs of local caregivers
- Provide individual counseling and support to local caregivers
- Maintain appropriate documentation for all program activities
- Develop and implement support groups
- Develop and implement educational workshops to support caregiver health and wellness
- Advocate for local caregivers through practical assistance and interventions.
- Provide appropriate referrals and resources
- Evaluate and monitor program activities to identify caregivers’ needs and goals, to track program quality and relevance, and to develop program strategic plans
- Work in partnership with members of the Seniors’ Department, and other teams at Warden Woods, to integrate the Caregiver Support Program and program participants throughout the organization

**Qualifications**:

- Degree in Social Work, or a related field, or equivalent learning from experience
- Minimum 2 years’ experience related to Senior’s programming, people with disabilities, caregiving, personal support work or another role relevant to this work
- Minimum 2 years’ experience supporting others, individually and in a group, in a community context
- Demonstrated ability to facilitate workshops, support groups and presentations
- Demonstrated ability to form and maintain effective partnerships for coordinated service delivery
- Ability to use Microsoft Office Suite with proficiency
- Valid driver’s license is an asset
- Proficient communication skills - oral and written English. Language capacities other than English are a definite asset.
- Understanding of issues and systems affecting seniors and people with disabilities
- Understanding of issues and systems affecting caregivers
- Sound judgement and demonstrated ability to respond appropriately to crisis situations
- Resourceful, personable, and able to make connections across a range of demographics
- Ability to work in a community-based environment using an inclusive and client-centered approach
- Availability to work evenings and weekends as required by program or community needs
- Must be legally entitled to work in Canada

**Compensation and Hours of Work**:

- $44,000 per annum
- Shift Hours: Monday-Friday 35 hrs/week
