**POSITION**: Case Manager, Community Withdrawals
**LOCATION**:Halton Region

**SUMMARY**
Provides clinical services to clients seeking withdrawal management support. Services will include withdrawal planning, symptom monitoring and counselling support, comprehensive clinical assessment and treatment planning and transitional short term case management. Services will be delivered throughout the Halton Region.
**ESSENTIAL DUTIES AND RESPONSIBILITIES**
- Direct Clinical Services - Withdrawal Management Counselling_
- co-ordinates and participates in the direct clinical care of individuals seeking withdrawal management services
- conducts initial withdrawal symptom screenings and comprehensive withdrawal management assessment and treatment planning
- in consultation with the client and any additional health care providers (as appropriate) develops a plan for safe withdrawal
- provides education to clients and support persons about the withdrawal process
- regularly monitors and assesses client withdrawal symptoms
- provides office based, community based and outreach support for clients within the Halton Region
- provides individual withdrawal management counselling to address the clients’ needs
- Direct Clinical Services - Transitional Case Management:_
- assesses client needs through the use of diagnostic tools (including but not limited to GAIN and OCAN) and structured interviews
- in consultation with the client, develops an individual Treatment Plan for the client
- facilitates referrals to appropriate support services and agencies according to the clients’ needs
- provides transitional short-term addiction or concurrent disorders transitional case management support
- conducts appropriate case management services with respect to the client’s Treatment Plan
- Direct Clinical Services - General:_
- provides consultative services to community agencies, professionals or individuals as required
- creates and maintains accurate and complete client records according to guidelines established by ADAPT, the MOHLTC and the Community Concurrent Disorders Program (CCDP), including client registration and tracking of services delivered
- provides crisis intervention support as needed
- provides motivational, supportive counselling and case management services to CWMS clients
- conducts case conferences and consultations with treatment providers, including physicians, other community organizations and others as appropriate
- conducts group sessions as required
- advocates on behalf of clients
- Non-Clinical Services:_
- provides knowledge transfer / staff education for ADAPT, CCDP staff and other allied services professionals, with a focus on expanding staff knowledge about withdrawal management, addictions, mental health and concurrent disorders
- provides input into the operational and strategic planning related to ADAPT programs as required
- may participate in community education and public awareness initiatives as required
- assists in the promotion of CWMS, CCDP, ADAPT and its programs to other agencies and the general public
- assists in maintaining a high level of open and effective communications and co-operation within the organization
- provides other services according to the conditions of program funding
- participates in clinical supervision and performance evaluations as required
- participates in committees as required
- participates in professional development activities as required
- participates in clinical supervision sessions as required
- participates in meetings as required
**EDUCATION**
University degree/ diploma in Social Work/ Social Service and/ or Health related field plus Addiction Studies certification
**LICENSES, CERTIFICATIONS AND CREDENTIALS**
- Registered Social Service Worker is an asset;
- Current CPR and First Aid certification is required.
- ASIST training or equivalent is required.
- CPI training is required.
- GAINS certification is an asset.
**RELATED WORK EXPERIENCE**
- 1-3 years of relevant experience;
- Experience in:
- Individual, family, and group therapy
- Community development and networking
- Record keeping and file management
**REQUIRED SKILLS AND KNOWLEDGE**
- Advanced knowledge of substances and withdrawal symptoms
- Advanced knowledge of psychotropic medication.
- Knowledge of mental health issues and related legislation, specifically Ontario’s Mental Health Act and PHIPA.
- CBT counselling skills is an asset.
