**Summary**:
The Youth Outreach Worker (YOW) will work closely with High-Risk Vulnerable Youth HRVY) in the Coquitlam community in connection and collaboration with community partners including School District 43.

The YOW is required to provide 1:1 direct support working with HRVY clients to assess needs and will develop and implement a case management plan for the individual and groups and families when and where necessary. Utilizing a range of counselling methods and techniques, the YOW will provide, and conduct skill building interventions, support, and direct counselling, to address areas of need and concern for the individual client. The YOW will provide case management services, connection, and support to the individual in alignment with their individual plan and goals. The YOW will also engage the Community and Child and Family At-Risk Support Table (CFAST) at their bi-monthly meetings informing and connecting community leads on the activities of program and service delivery.

This is an HSA Bargaining Unit Position and requires Union membership.

**Key Duties and Responsibilities**:

- Gathers information relevant to youth’s problems by interviewing, observing behaviour, meeting with caregivers/service providers to assess and determine the level of risk.
- Conducts structured analysis of the information gathered to provide an assessment and determine program eligibility.
- Develops and implements counselling plans and intervention using a variety of therapeutic counselling techniques drawn from theoretical framework such as family systems, solution-focused, narrative, psycho-dynamic group work and advanced group counselling techniques to resolve the assessed problems.
- Provide a safe and supportive environment for youth who identify as vulnerable and/or high risk working in partnership and collaboration with community partners including School District 43, YMCA Alternate Suspension Program, and local authorities.
- Support the youth to identify goals and desired outcomes in their engagement with program service delivery and provide prevention support for youth closely associated with gang involvement, violence, or gun activity.
- Working closely with the School District the YOW will be required to implement an attendance strategy to increase school attendance as part of their goals and outcomes.
- Work collaboratively with the family and others who support the individual to connect and inform necessary parties of their development through support programming.
- Monitor youth’s progress and provide follow-up planning.
- Evaluate the effectiveness of counselling plans, reports youth’s progress and discusses case management and related concerns with therapist, social workers, peer, or other professionals.
- Provides case management services and consultation to other service providers. Provides information on and refers youth to other community service providers, resources, or professionals as required.

**Qualifications Required**:

- Minimum bachelor’s degree in related human/ social service field related to child and youth care or community support work.
- Minimum 2 years’ recent experience supporting children/youth who are considered high risk, and/or experience working with youth with or at risk of gang involvement. An equivalent combination of education, training and experience will be considered.

**Skills and Abilities**:

- Ability to work effectively with individuals/families, volunteers, other service providers including contractors, community partners and other stakeholders related to food security projects.
- Excellent interpersonal, oral, and written communication skills.
- Strong analytical, problem-solving, and decision-making skills.
- The ability to work both collaboratively and independently.
- Strategic thinker with the ability to identify and develop new approaches to challenges in your work.
- Trauma informed practice and approaches are considered an asset.
- Clean driving abstract record: use of personal vehicle is required.
- Current First Aid (or willing to attain)

**Location**:Tri Cities (Coquitlam, Port Coquitlam, Port Moody)

**Hours: 35 Hours Per Week **Monday to Friday (with some flexibility in the evening)
