**About Us**:
Medical Clinic at Southpointe is a veteran owned and operated integrated health care clinic in the heart of Fort Saskatchewan. We are comprised of a diversified team of Physicians, Nurse Practitioners, a General Surgeon, a Registered Nurse, a Chiropractor, Massage Therapists, a Behavioural Health Consultant and an amazing administration team. Alongside family healthcare and clinical integrated services, we also run weekly Well Women's Clinics for all related women’s health and a Veterans Support program to assist with their complex needs.

**About the Role**:
The Clinical Care Manager uses their clinical knowledge and skills to support the patient

and provider through the various stages involved in the care process. With a new

and developing model, the role of the Clinical Care Manager will change and evolve.

Focused primarily on clinical care management for Veterans Affairs Canada (VAC), the right individual will have the ability to help develop the scope of work and grow their experience. Continuous learning and feedback opportunities will be provided for all members of the Medical Clinic team.

**Highlights of the Role**
- Full-time (37.5 hours per week)
- Flexible days, evenings, and weekends
- Fort Saskatchewan, Alberta (25 minutes northeast of downtown Edmonton)

**Expected Outcomes**:
1. **Optimized Patient Care Coordination**: The Clinical Care Manager will streamline patient care processes, ensuring smooth transitions between various healthcare providers and departments within the clinic. This includes efficient scheduling of appointments, consultations, diagnostic tests, and follow-up care.

2. **Enhanced Patient Satisfaction**: By serving as a point of contact and resource for patients, the Clinical Care Manager aims to improve overall patient experience. This involves addressing patient inquiries, concerns, and needs promptly and effectively, resulting in increased satisfaction with the clinic's services.

3. **Improved Clinical Workflow Efficiency**: The Clinical Care Manager will identify bottlenecks and inefficiencies in the clinic's workflow and implement strategies to optimize productivity. This may involve standardizing procedures, utilizing technology for documentation and communication, and coordinating staff schedules to ensure optimal utilization of resources.

4. **Effective Communication and Collaboration**: Facilitating effective communication among healthcare providers, support staff, and patients is a key outcome for the Clinical Care Manager. By fostering a collaborative environment, the facilitator ensures that all team members are informed, engaged, and working towards common goals, ultimately leading to better patient outcomes.

5. **Enhanced Patient Education and Empowerment**: The Clinical Care Manager plays a crucial role in providing patients with information about their health conditions, treatment options, and self-care strategies. By empowering patients to take an active role in their healthcare decisions and management, the facilitator contributes to improved health literacy and patient outcomes.

6. **Optimal Resource Utilization**: The Clinical Care Manager should strive to optimize the use of clinic resources, including staff time, equipment, and supplies. This involves efficient allocation of resources based on patient needs, minimizing waste, and identifying opportunities for cost savings without compromising quality of care.

7. **Quality Improvement Initiatives**: Participating in quality improvement initiatives and monitoring key performance indicators are essential outcomes for the Clinical Care Manager. By collecting and analyzing data on clinic processes, patient outcomes, and satisfaction levels, the facilitator can identify areas for improvement and implement evidence-based practices to enhance overall quality of care.

8. **Compliance with Regulatory Standards**: Ensuring compliance with relevant healthcare regulations, accreditation standards, and best practices is a critical outcome for the Clinical Care Manager. This includes maintaining accurate documentation, adhering to privacy and confidentiality protocols, and implementing policies and procedures to mitigate risks and ensure patient safety.
