**Position Overview**:We are seeking a compassionate and skilled Counsellor to join our multidisciplinary team. In this role, you will serve as a Hospital Liaison, working primarily within hospital settings to ensure a smooth transition for clients from in-patient and withdrawal management programs to PAARC community services. You will provide clinical treatment planning, therapeutic counselling, case management, and community education, all while upholding the highest standards of care and professionalism.

**Key Responsibilities**:

- **Clinical Assessment & Treatment Planning**: Conduct comprehensive assessments, including risk and psychosocial assessments, and develop individualized care plans in collaboration with clients.
- **Therapeutic Counselling**: Provide ongoing therapeutic counselling, case management, and client support, facilitating both individual and group sessions as required.
- **Hospital Liaison**: Act as a bridge between hospital in-patient services and PAARC’s community programs, ensuring clients experience a seamless transition.
- **Client Advocacy**: Support clients in understanding their rights and treatment options, and advocate for their needs within the healthcare system.
- **Administrative Duties**: Maintain accurate and timely documentation, participate in team meetings, and contribute to the continuous improvement of the program.

**Qualifications**:

- Master’s degree in human services (completed or in progress) from an accredited institution.
- Certification in Stage Screening and Assessment.
- Experience in substance abuse counselling, with knowledge of withdrawal management and trauma-informed care.
- Familiarity with anti-racism, anti-oppression frameworks, and a commitment to these principles.
- Certifications in Non-Violent Crisis Intervention, CPR, and First Aid.
- Strong communication skills, both verbal and written.
- Ability to work effectively as part of a multidisciplinary team.

**Working Conditions**:

- Flexibility to work outside regular business hours as needed.
- Travel within Peel Region and the Greater Toronto Area (GTA) to meet clients in various settings.
- Ability to work in different environments, including hospital settings and community locations.
- Must be comfortable with extensive keyboarding and use of office technology.

Pay: $56,242.00-$65,616.00 per year

- Peel, ON: reliably commute or plan to relocate before starting work (required)
