**About Continuity Care**:
Continuity Care is community based, not-for-profit organization created by a group of dedicated parents, siblings and concerned citizens. In 1996, Continuity Care was incorporated and registered as a Canadian charity.

Our Mission Statement is "supporting families in Manitoba to plan for an enriched qualify of life for their family member with an intellectual disability".

Continuity Care currently has approximately 200 members and more than 740 individuals, families and organizations on our mailing list.

**Position Summary**:
The **Member Support & Outreach Facilitator** (MSOF) provides support and assistance to families to plan for the needs of their family member with an intellectual disability. The MSOF works closely with the Executive Director, and in collaboration with service providers, government programs and community members. All employees of Continuity Care work within the Mission and Values of Continuity Care, in accordance with legislation and with the policies governing the organization.

**Qualifications**:
**Education**:

- Post secondary degree in the disability or human services field; Social Work preferred
- Valid registration with the professional association, if applicable
- Courses and workshops specific to the field of disability, person centered planning (specifically PATH & MAP’s), interpersonal skills, communication are an asset.

**Knowledge and Experience**:

- A minimum of 5 years of experience within the disability sector
- Knowledge and understanding of people with disabilities and the values and philosophies that are the foundation of person centered and family centered practice
- Strong interpersonal skills including conflict resolution and mediation
- Knowledge of the Adults Living with an Intellectual Disability Act (formerly Vulnerable Persons Act), Personal Health Information Act (PHIA) and other relevant legislation
- Knowledge of community resources and relevant systems
- Ability to research and explore resources in the community
- Experience facilitating groups and with public speaking
- Ability to communicate effectively both orally and in writing
- Ability to be flexible and work within a non-conventional schedule for the benefit of members
- Familiar with virtual platforms, such as Zoom and MS Teams

- Completes successful Criminal Record Check including the Vulnerable Sector Search, Child Abuse Registry and Adult Abuse Registry checks
- Has access to a vehicle and possesses a valid driver's license

Expected hours: 37.5 per week
