Case Coordinator, Addiction Treatment Programs

Hiring for two positions:
- One: full time permanent, weekday evenings 4 pm - midnight. Pension at start, benefits after three months. Position in our Bed-Based Treatment Program.
- Retention incentive in addition to pay.
- Position Description:_ Case Coordinators, working as part of our Addiction Treatment team, work with men in our residential program, as well as providing continuing care to individuals prior to residential admission, after completion of residential programs and requiring addiction counselling in the community. The Case Coordinator performs a range of activities including: completing intakes with clients; leading groups; individual counselling; new client entry into Catalyst; maintaining accurate case notes in Catalyst; supporting colleagues to manage the wait list for our Residential Services; pre-treatment care planning; collection of necessary client information to support the intake process; collaboration with community and health partners; assessments; brief interventions; individual counseling; group facilitation; case management functions; working with our Aftercare staff to provide supports for post-residential services; and teaching the above processes and skills to student placements.
**Knowledge, Skills and Abilities**
- Knowledge of issues related to addiction, concurrent disorder, mental health, trauma and recovery.
- Knowledge and understanding of community housing, addiction and mental health services.
- Proficiency in the use of Catalyst required, as well as general computer proficiency.
- Experience working in a residential setting with men living with addictions and/or mental health.
- Experience utilizing various approaches to housing and substance dependence.
- Extensive knowledge and skills in building rapport, assessment, supportive counseling, case management and brief-intervention.
- Knowledge of the social determinants of health.
- Knowledge, understanding and sensitivity to the complexity of issues arising for individuals who have experienced homelessness, addictions, poverty and/or mental illness.
- Ability to work collaboratively within and across interdisciplinary teams and community partners.
- Ability to work with clients presenting with a wide range of addiction and mental health issues.
- Demonstrated flexibility, sound judgment, initiative and creativity.
- Experience working within an anti-oppression and trauma informed framework.
- Excellent interpersonal and communication skills.
**Qualifications**
- University degree in a related field OR equivalent education and related work experience, with a preference for social work, psychotherapy, or formal addiction and mental health programs.
- Registration with a regulated professional college an asset.
- Demonstrated experience working with men with addictions or concurrent disorders.
- Demonstrated experience in case management.
- Demonstrated experience in motivational interviewing and other counselling and therapeutic approaches.
- Demonstrated experience with group development and facilitation.
- Additional training in screening, assessment and treatment related to addictions an asset.
- Ability to work in a language in addition to English is a definite asset.
**Salary**: $45,138.00-$54,204.00 per year
