Right now we are looking for a **Addictions Counsellor** to work with our **Mental Health & Addictions Program** team**.**

**Hourly Rate**: $27.38 - $33.35

**Location**:Humber River Hospital - all sites

**Responsibilities**:

- Completes screening and a detailed addiction assessment for all newly referred patients.
- Supports the patient during all phases of recovery from usage to abstinence.
- Plans, implements, and evaluates individual and/or group treatment in accordance with the program philosophy and objectives.
- Provides individual counseling and goal setting to assigned patients.
- Provides education to patients, families and significant others on various aspects of chemical dependency.
- Develops and coordinates the discharge plan in conjunction with the treatment team including connection/referral to appropriate internal and/or community resources.
- Completes patient documentation, workload measurements and data requirements in accordance with Hospital, Ministry of Health, CLHIN and departmental standards.
- Responds to clinical crisis in an effective and efficient manner.
- Other duties as assigned

**Requirements**:

- Possesses an Addiction Counselor Diploma and/or is regulated health professional with the required addiction knowledge and experience.
- 2 years experience providing addiction counseling required.
- Experience working with individuals with coexisting mental illness preferred
- Demonstrated experience - individual and group counseling - chemically dependent individuals.
- Excellent interpersonal and communication skills required in order to facilitate effective communication with patients, families and team members
- Demonstrated excellent written and verbal communication skills.
- Extensive knowledge of community based addictions resources.
- Knowledge of adult teaching principles is an asset.
- An awareness and Knowledge/experience in Trauma Informed Care preferred
- Ability to respond rapidly and decisively to crisis situations.
- Knowledge of DATIS is an asset
- Experience, familiarity and comfort with technology-assisted healthcare delivery, specifically video conferencing using MS teams and Zoom
- Ability to use mobile technologies such as smartphones and tablets to connect patients, preferred
- Technology literacy and usage skills preferred
- Experience with communication and assessment skills during video appointments, preferred
- Demonstrated ability to work well individually and within a team structure.
- Demonstrated cross cultural awareness and sensitivity.
- Demonstrated ability to prioritize a varied workload.
- Experience, familiarity and comfort with technology-assisted healthcare delivery, specifically video conferencing using MS teams and Zoom
- Ability to use mobile technologies such as smartphones and tablets to connect patients, preferred
- Technology literacy and usage skills preferred
- Experience with communication and assessment skills during video appointments, preferred
- Excellent attendance and discipline free record is a requirement
