**Position Summary**

Under the direction of the Jordan’s Principle Program Manager, the Jordan’s Principle Advocates must have extensive experience working in and with the First Nations children, youth and families. The Jordan’s Principle Advocates must be able to work both independently and within a team environment to advocate and support First Nations children youth and families while they navigate various systems residing in Winnipeg.

**Key Responsibilities**
- Supporting First Nation children, youth and families who require advocacy while navigating multiple services.
- Maintaining working relationships with key contacts in the provincial and federal government departments, organizations, and agencies.
- Communicating with the families for health care specific interventions, plans, learning needs and how they will be met.
- Supporting families who require multiple support and connecting to appropriate resources
- Facilitating and planning network meetings with various organizations and provincial and federal agencies to support the best interest of First Nation children, youth, and families.
- Communicates to the families, as necessary for referral information that will be given to appropriate community organizations and service provider.
- Collaborates with and make referrals to appropriate service providers and or programs for resolution of issues of concern.
- Coordinate the activities with other health and social disciplines in implementing the individual discharge and follow up plans for selected children/adults (Nursing Stations, Health Centre’s, NIHB, Patient Transportation Unit, Home Care, Education, Child and Family Service Agency, etc.).
- Maintain confidentiality according to PHIA standards.
- Consult, collaborate, and provide reports and data to FNIHB.
- Other duties as assigned.

**QUALIFICATIONS**

**Education and Experience**
- Post-Secondary degree in Human Services (Native Studies, Social Studies, etc.)
- Equivalent combination of education and a minimum of five (5) years’ direct experience working in the child welfare system, working with First Nation individuals with complex medical conditions, disabilities and may be considered.
- Superior verbal, listening written and inter-personal communication skills.
- Demonstrates problem solving, time management and organization skills.
- Experience working with child and family services and have an understanding various legislations pertaining to the best interest of the child.
- Experience working effectively with First Nation children, youth, and families.
- Knowledge and familiarity of the programs and resources available to First Nations citizens who are residing in the urban area.
- Knowledge of First Nation culture, traditions, attitudes, and socio-economic conditions
- Knowledge of and/or experience in collaborative, integrated program, and community planning.
- Experience in using Client Centered Case Management as a service delivery tool.
- Ability to establish and maintain positive and effective work relationships with coworkers, clients, and external agencies.
- Ability to work closely with a variety of stakeholders including health care executives and professionals, senior levels of government and community members.
- Ability to use Microsoft Office software, Word, Excel, and Outlook.
- Knowledge of First Nations culture and fluent in a First Nations language would be an asset.
- Adhere to AMC policies, and procedures with specific attention to attendance, and professional ethics.
- Must have a valid Class 5 driver’s license and a reliable vehicle.
- Must have a current satisfactory Criminal Record Check, Clear Child Abuse Checks.
