Nursing Coordinator

We seek a dedicated to finding a a Nursing Supervisor (LPN, RPN, RN) to join our team. This role is ideal for someone passionate about making a difference in the lives of others by delivering exceptional in-home care services.
As this is a Mat Leave coverage position, extensive orientation will be provided.
**Responsibilities**:
- Conduct home visits to provide nursing care in alignment with individualized care plans.
- Perform health assessments, monitor changes, and document observations.
- Educate clients and families on health management, preventive care, and equipment use.
- Ensure care plans, forms, and documentation are up-to-date in the ERP system.
- Support and train healthcare staff in safe lifting, transfers, and catheter care.
- Conduct environmental assessments and make equipment or community resource recommendations.
- Collaborate with directors of care, social workers, and other professionals to enhance client outcomes.
- Assist with scheduling and provide feedback to improve services.
- Contribute to marketing and outreach initiatives to increase awareness of Home Care West.
- Active LPN/RN/RPN license in British Columbia.
- Current CPR and First Aid certification (or willingness to obtain upon hire).
- Experience in home care or community nursing (preferred).
- Strong interpersonal, organizational, and communication skills.
- Ability to work independently and prioritize tasks effectively.
- Reliable transportation and a valid driver’s license.
- Proactive thinker and contributor
- Understands the importance of relationship building
- Excellent English communication skills, both written and verbal
- Extremely well-organized and detail orientated
