**Responsibilities**:

- Accompanies the Physician during patient encounters and documents relevant information in real-time including the reason for the visit, medical history, review of systems, physical examination findings, diagnoses, and treatment plans.
- Ensures comprehensive and accurate documentation while adhering to established guidelines and templates.
- Organizes patient charts and ensures they are complete, up-to-date, and well-structured.
- Assists in creating templates or standardized forms to streamline the documentation process.
- Reviews and verifies the accuracy of recorded information, making corrections when necessary.
- Liaison between the Physician and other members of the healthcare team and will convey critical information to nurses, and other team members, ensuring smooth communication and coordination of patient care.

Qualifications:

- Post-secondary education, such as a certificate or diploma in medical office administration, health sciences, or a related field.
- A strong understanding of medical terminology.
- Possess knowledge of anatomy, physiology, medical conditions, procedures, and medications to accurately transcribe and document patient encounters.
- Experience navigating EHRs, typing efficiently, and using various software programs for data entry and document management.
- Excellent attention to detail to accurately capture and record patient information.
- Ability to listen attentively, transcribe information in real-time, and identify and correct any errors or inconsistencies in documentation.
- Strong verbal and written communication skills and able effectively collaboration with healthcare providers and other members of the healthcare team
- Good organizational skills, the ability to prioritize tasks, and the capacity to work efficiently under pressure.
- Maintains confidentiality and adhering to ethical guidelines is paramount
