Reporting to the Director of Care, the Infection Control Practitioner is responsible for providing oversight and leadership to the infection, prevention and control (IPAC) program at Valleyview Home. This particular role will have primary accountability for the IPAC program and will provide oversight and leadership to ensure regulatory compliance and implementation of clinical care standards and best practices related to infection prevention and control. The Infection Control Practitioner will ensure that Valleyview Home is meeting its obligations in compliance with Fixing Long-Term Care Act, 2021 (FLTCA) legislation and amendments, Valleyview Home policies, Public Health requirements and standards and IPAC best practices.

**MAJOR TASKS**:
Infection Surveillance - 15%
- Develops a surveillance plan based on the population served, services provided and previous surveillance data.
- Monitors Ministry of Health and Ministry of Long-Term Care reportable indicators.
- Contributes to the selection of additional indicators to monitor based on the projected use of data (i.e., external benchmarking, internal trending).
- Prioritizes surveillance responsibilities.
- Generates and analyzes surveillance data using rates, denominators and risk stratification as appropriate to the indicator on a monthly basis and to meet internal and external reporting requirements.
- Recognizes variances in indicators, evaluates and compares surveillance data to either internal or external data sources.
- Reports epidemiologically significant findings to appropriate parties.
- Performs other related duties as may be assigned.

Infection Prevention and Control Standards of Practice - 40%
- Assists care teams and service teams to apply/adapt infection prevention and control standards and principles in their areas and create program specific protocols.
- Participates in facility programs such as hand hygiene, influenza vaccination, created to implement infection prevention best practices and standards, and advances the facility goal to improve resident safety.
- Works collaboratively with internal and external practitioners including Public Health to regularly review and revise practices/protocols to ensure integration of most recent evidence based best practices.
- Applies knowledge of infection prevention and control principles to provide consultation on new/ongoing procedures and procurement of new or evaluation of current resident care equipment.
- Provides expertise to committees, advisory groups and task teams, both internal and external.
- Collaborates with the Joint Health and Safety Committee and Occupational Health and Safety Coordinator in reviewing programs that pertain to staff safety.
- Maintains current knowledge of regional and national standards and recommendations related to infection prevention and control, in the long term care setting.

Case Consultation and Outbreak Management - 15%
- Reviews and assesses clinical presentations to determine risk of communicability to other residents and staff.
- Provides consultation and guidance regarding care of residents with infectious conditions to minimize transmission.
- Develops, implements and assesses measures to contain organisms of epidemiological importance.
- Investigates and manages outbreaks identified through surveillance; leads the outbreak management process by convening stakeholders, reviewing accountability requirements of leaders and staff to adhere to appropriate infection safety measures relevant to contain the outbreak.
- Advises on measures to contain outbreak, and communicates, educates and coaches care teams in the adoption of the measures.
- Throughout the duration of the outbreak, monitors cases, reports and consults, as necessary with the appropriate stakeholders.
- Completes outbreak summary reports and provides to Public Health.
- Investigates and manages sentinel events.
- Liaises with the Public Health Unit in emerging or urgent situations of outbreak management.

Education and Training Healthcare Workers - 20%
- Complies with and instructs others on resident infection safety initiatives, e.g., hand hygiene protocols, routine practices.
- Serves as an educational resource for infection prevention and control.
- Performs continuous assessment of the educational needs of staff relate
