**Duties include**:

- Serving as a trusted advocate and liaison between clients, caregivers, healthcare professionals, office team, facilitating seamless and open communication and coordination of care.
- Must demonstrate excellent oral and written communication and the ability to listen effectively.
- Collaborate with clients and their families with an in-home assessment to identify client needs and develop personalized care plans tailored to their individual preferences and goals.
- Facilitate introductions between caregivers and clients.
- Monitor clients' progress and regularly adjust care plans as needed to meet changing needs and preferences.
- Provide ongoing support and guidance to clients and their families, addressing concerns and resolving issues with empathy and professionalism.
- Conduct quality assurance visits, in the field training to ensure the highest standard of care is being provided to ensure high quality of care, client satisfaction and retention as well as opportunities to increase service hours.
- Maintain accurate and up-to-date documentation of client information, care plans, caregiver visit notes in compliance with agency policies and regulatory standards.
- Flexibility in handling unexpected emergencies or crises (such as a sudden deterioration in a client's condition) requiring quick decision making and adaptability.
- Effective time management prioritizing urgent cases while multitasking appointments and patient needs, and ensuring timely follow-ups.

- Ability to manage multiple tasks and priorities effectively while maintaining attention to detail.
- Previous experience in case management, care coordination, or a related field, preferably in a home care or healthcare setting.
- Minimum of 5yrs nursing experience is preferred.
- Proficient computer skills including scheduling software (Caresmartz360, Word, Outlook, Canva, etc.)
- Valid driver's license and reliable transportation is a requirement.
