**Job Summary**

**Emergency Shelter Program & Case Management**

The Housing, Community & Data Worker (HCDW) will work in conjunction with our emergency shelter program to support participants who are experiencing homelessness or are at risk of homelessness by practicing diversion and engaging community supports. The HCDW will support participants to help them to transition to safe and secure housing in a case management capacity. Recurrence prevention strategies will be offered to all participants to further support their transition(s).

**Community Meal Program**

The HCDW will work directly with our Community Meal Program in conjunction with the Food Service Department and offer food security support and intentional community. The HCDW will provide support to participants who are facing challenges including poverty, mental health, addictions, abuse, and social isolation with an emphasis on identifying those who are homeless or at risk of homelessness or struggling with food security.

**Data Program**

The HCDW will work directly with our frontline staff and management team to collect and analyze data related to participant stays, services and programs provided by The Lighthouse. The HCDW will work with all departments to ensure that accurate data is collected in a timely fashion for reporting to the multiple funding source requirements. The DHW will also run reports to facilitate internal reporting statistics and assist in the creation of new data collection systems as required.

**Accountability**: Able to affirm to the vision, mission and beliefs of The Lighthouse by demonstrating commitment to our organization's values of flexibility, adaptability, compassion and dignity with sensitivity to the issues related to poverty, homelessness, mental illness, addictions and developmental disabilities with a commitment to a discrimination-free environment.

Reporting to the Operations Manager, or such other persons as required by the organization.

**Qualifications**
- A person who has a passion to end homelessness and is a good role model.
- College diploma or University degree in Social Service or Human Services, or equivalent experience.
- Experience working with people experiencing chronic homelessness or who have challenges in obtaining and maintaining housing and within a diverse community.
- Experience in a communal setting and some food service experience.

**Skills Required**:

- High standard of integrity, judgement and confidentiality
- Demonstrate ability to multitask, prioritize, organize, meet deadlines, maintain the responsibilities of a full caseload and provide strong statistical data analysis skills.
- Solid computer and internet skills; Google Docs, Google Forms, Google Sheets, Excel etc.
- Demonstrated conflict resolution, advocacy skills, understanding of harm reduction and person centred approach.
- Demonstrated trauma informed support skills.
- Proven experience to work independently and as part of a team.
- Ability to collaborate with community partners to meet the needs of participants.
- Working knowledge of community resources including but not limited to health/mental health, counseling, addictions, employment, education, financial, recreation etc,
- A satisfactory Vulnerable Sector Criminal Records check is required.

**Assets**
- A valid Ontario driver’s license, liability insurance and access to vehicle an asset.
- Working knowledge of Ontario Works and Ontario Disability Support Program is an asset.
- Knowledge of the County Database (HIFIS), SPDAT’s and the Housing First Program is an asset.
- Training in nonviolent crisis intervention, first aid/CPR, first aid mental health or relevant equivalents is an asset and will be required during the time of your employment.
- A Safe Food Handlers Certificate is an asset

**Hours of Work**
- Full time position 30-40 hours per week primarily weekdays, some statutory holidays required, some flexibility of some evenings and weekends only as needed
- Salary of $33,000 based on a 30 hour work week or $44,000 based on a 40 hour work week including benefits
