Exciting career opportunity- Transformation Lead- 2-Year Contract / Secondment for Timiskaming Area Ontario Health Team-:
- Ontario Health Teams (OHTs) are a new model of health-care delivery that puts patients, families and caregivers at the center of the health-care system. OHTs formally bring together groups of providers and organizations that will be clinically and fiscally accountable for delivering a full and coordinated continuum of care to a defined population. Health service providers from across the Timiskaming district and surrounding area have recently been approved as an Ontario Health Team (“Timiskaming Area OHT”).- The Transformation Lead will collaborate with all members of the Timiskaming Area Ontario Health Team (TAOHT) to ensure the successful development and implementation of the Ontario Health Team structure, key goals and objectives. The Transformation Lead will support the work of the OHT, ensuring deliverables and milestones are achieved, processes for collaboration are developed, a communication strategy is established, and that the OHT is in alignment with direction from the Ministry of Health and Ontario Health. The Transformation Lead will be under the direction and support of the leadership of the TAOHT’s Collaboration Council.- Other key responsibilities include developing processes to enable collaboration with all partners and members including Councils to be established (i.e., Patient and Family Collaboration Network, Primary Care Council and Board), conducting research, compiling briefings, presentations, and completing data analysis. POSITION RESPONSIBILITIES:
- Develop and monitor project plans and timelines to ensure the work of TAOHT meets all internal and external milestones.
- Facilitate the development and execution of a work plan to achieve desired outcomes, deliverables and milestones.
- Support the Collaboration Council by conducting necessary research and environmental scanning to ensure stakeholders have the perspective and information required to make sound decisions.
- Provide regular updates and recommendations to the Collaboration Council on the current state of strategic and project planning, including reports on the progress of the Working Groups and Advisory Committees.
- Ensure Indigenous communities and partners, francophone groups, and physicians are consulted, engaged and informed about TAOHT’s goals and objectives.
- Provide leadership in establishing working groups/advisory tables and provide ongoing support by establishing and adjusting work plans, coordinating meetings, and collaborating with chairs to identify needed resources.
- Report on all MOH and OH deliverables and outputs, such as, quality, financial and performance indicator submissions.
- Coordinate, monitor and report on multi-year business planning.
- Monitor and report on the TAOHT legislative compliance.
- Identify, mitigate, and assist in managing risks associated with strategic and overall business planning.
- Responsible for any direct reports.
- Act as a key point of contact for coordination of all system partners, planning, reporting and communication activities.
- Engage with neighboring OHTs to stay aligned and leverage synergies.
- Collaborate with member analytics and data/planning teams where possible to advance the work of the Collaboration Council and members where required.
- Liaise with Ontario Health / Ministry of Health and participate in regional and provincial OHT meetings on behalf of the OHT, as applicable.
- Support the collection of quality and performance indicators.
- Act as the primary contact between the OHT and the Ministry, and stay current on MOH direction.
- EDUCATION AND EXPERIENCE REQUIREMENTS:
- Degree in public administration, business administration, healthcare administration or project management, or equivalent experience. an asset in a related discipline (i.e., healthcare administration, business administration).
- Minimum 3+ years of experience in project leadership in healthcare environments such as hospitals, long-term care, primary care, government or private sector required.
- Experience with leading and managing the implementation of strategic and operational directions and goals.
- Extensive knowledge of provincial health care structures, including Indigenous provincial health structures, processes, funding mechanisms and current priorities is strongly preferred.
- Understanding of various health care sectors including primary care, home care, mental health, long term care, acute care, community health and community services.
- Working knowledge of financial modeling and financial analysis is an asset.
- Working knowledge of quality improvement processes and data/information analysis.
- Experience working with cross-functional teams, including group facilitation.
- Indigenous Cultural Safety course completion an asset.
- Bilingualism (French/English) is preferred.
