**Career Development and Case Work Manager - 1.0 FTE - Opportunities Fund for Persons With Disabilities**

**Responsibilities**:

- Actively recruit and assess applicants for employment training eligibility for youth with disabilities;
- obtain participant employment history, presenting disabilities, educational background and career goals;
- facilitate participant care by assessing employment barriers; developing, monitoring, and evaluating case plans and progress to overcome barriers and facilitate interdisciplinary approaches through referral, advocacy and service navigation;
- identify barriers to employment and assist participants with such matters as job readiness skills, job search strategies, writing résumés, preparing for job interviews and one to one training support;
- facilitate workshops in accordance with ESDC contracted requirements
- liase with community partners for referrals
- manage participant budget to ensure participants are receiving the right purchased supports and services at the right time; assess need for assistance such as rehabilitation, financial aid or further training and refer clients to the appropriate services;
- plan programs of assistance for participants including referral to agencies that provide financial assistance, legal aid, housing, medical treatment and other services
- communicating with family supports
- finding external employment placements
- adhering and meeting expected outcome results of return to school or employment
- report writing
- data tracking

**Education: Social Services Diploma or Degree**

**Transportation: Must have vehicle to meet with participants**

**Job Types**: Full-time, Fixed term contract
Contract length: 97 weeks

Pay: $35.00 per hour

Expected hours: 37.5 per week
