**Job Description - Youth Reaching Home Worker**

**Position Summary**:
“The **Youth Reaching Home Support Worker **assists youth in navigating housing searches and transitions, providing individualized support, and liaising with community and governmental partners. This role requires a strong understanding of the Residential Tenancy Act, excellent advocacy skills, and comprehensive knowledge of community resources within the region. The Support Worker engages with youth to create and implement wellness plans, support their journey towards stable housing, and facilitate access to essential services. They work closely with the Ministry of Children and Family Development (MCFD), local community partners, and landlords to ensure smooth transitions into stable living environments, advocating on behalf of youth and empowering them to understand their rights and access necessary resources”. **Job Duties & Responsibilities**:

- Supporting with Housing searches and supporting in transition plans (Ex: food and groceries, driving for appointments and viewings, etc.);
- Knowledgeable about the BC housing, affordable housing, LEM ( ) resources in the community
- Knowledgeable about RTB (Residential Tenancy Branch) & RTA (R T Agreement) guidelines
- One-to-one support such as creating individualised wellness plans, including frequent/periodic follow-ups;
- Acting as a liaison with MCFD and have an understanding of the SAGE (Strengthening Ability and Journeys of Empowerment);
- Liaising with MSDPR, community partners and individual landlords on behalf of youth;
- A sound understanding of a culturally appropriate ways of approaching community needs and experience working with multi-barriered street entrenched homeless youth;
- Support youth in finding employment including training & education;
- Other duties as required.

**Qualifications**:
**Education and Experience**:

- Education in Child and Youth Care, or other Human Services;
- A minimum of 1 year of experience in working with Indigenous youth;
- A Driver’s License and a clean drivers’ abstract;

**Key Knowledge Areas**:

- Knowledge of effective outreach and engagement practices with Youth;
- Knowledge of the diversity of Indigenous cultures, identities, and lived history;
- Harm Reduction approaches with at Risk Youth;
- Extensive knowledge of youth serving resources and supports in Surrey and the Fraser Valley including career, educational, job, food, income, cultural, and recreation.

**Key Skills and Abilities**:

- Ability to manage multiple tasks and works well under pressure;
- Highly accountable to program, team, and youth;
- Team Player and able to work with a diverse range of people and personalities;
- Excellent general administration skills, including proficiency in Microsoft Office;
- Flexibility in work schedule and location;
- Able to travel and outreach through Surrey.
