**PRIMARY FUNCTION**:
Reporting to the Program Operational Director for Women and Children’s Programs (W+C), the Safety and Quality Improvement Project (SQIP) Coordinator will lead and support quality and patient experience related improvement projects throughout the program. The SQIP Coordinator will also provide support to patient and staff safety incident follow up. The incumbent will seek out new and innovative quality practices, help address environmental concerns and review/analyze/synthesize data and literature as it relates to projects that address safety, quality and the patient experience. Responsibilities will include leading initiatives on program Quality Improvement Plans (QIPs), ad hoc projects that arise in the portfolio and assisting with SAFE incident follow up and program reviews.

Working collaboratively with the entire W+C leadership team, the SQIP Coordinator will coordinate process and practice improvements that enhance quality/safety, operational efficiency, patient experience and program priorities such as EDI. Initiatives will aim to reduce errors, mitigate risks, improve the patient experience, maximize value and eliminate waste. This role will also conduct audits, administer surveys, create tools/guides as needed and ensure that adherence to best practice is based on evidence including data and literature.

The SQIP Coordinator will assist with gathering facts related to patient safety incidents (SAFE) in consultation with PSQR and program leadership, reporting findings to the appropriate stakeholders. Active membership on corporate and portfolio-level committees related to patient safety and quality improvement is required.

**RESPONSIBILITIES & DUTIES INCLUDE**:
**Quality Improvement Project Support**

Works collaboratively with leadership, physicians and front line staff in the development and maintenance of QI projects. This includes developing a standardized QI project format for the program utilizing tools and methods from PMI, Lean and Six Sigma; presenting evidence-based solutions for leadership to consider; data sourcing and synthesis; stakeholder engagement; ongoing reporting and management of project-related key performance indicators; utilizes standards tools to perform gap analyses, risk matrices and root cause analyses to support projects; manages project budgets and timelines

**Safety**

Supports SAFE report management by tracking and monitoring outstanding files and conducting initial follow up on SAFE reports. Participates in preparing and carrying out incident reviews. Liaises regularly with program leadership to ensure safety/quality related outcomes from SAFE investigations are discussed, coordinated, supported by management and implemented as necessary. Ensures that health and safety policies and procedures are operationalized and in accordance with best practices. Supports general Health and Safety programming by participating in committees/working groups across the program and hospital. Assists W+C Leadership with communication out to staff regarding changes in policies or procedures.

**Day-To-Day Operations**

Participates in health and safety hazard assessments, code/event staff debriefing, SAFE event reviews, risk assessments, etc. Monitors equipment to maintain in good working order and is safe for patients and staff to use. Compiles data needed for reviews, reports and ad hoc program needs and provides regular progress reports on QI project and safety initiative status. Executes other duties and responsibilities as required.

**Liaison with Other Hospital Departments**

Acts as a liaison with community partners and hospital departments to coordinate services, ensure timely transitions, and health and safety concerns are addressed. Collaborates with physicians, learners and other community partners within the region as required

**BASIC QUALIFICATIONS**:

- Requires completion of Community College Diploma (3 year program) or equivalent.
- Requires 1-less than 3 years with some practical or position related experience.
- 1-3 years practical or position related experience preferred.
- The SQIP Coordinator understands and is familiar with all pertinent KHSC policies and procedures including those relating to workplace conduct. Complies with the KHSC commitment to uphold the Workplace Conduct and Reporting of Inappropriate Conduct policy and behaves in a manner that is consistent with the guiding principles and expectations.
- Proven ability to attend work regularly.
- Satisfactory criminal reference check and vulnerable sector search required.
