The Haldimand-Norfolk Diabetes Program focuses on quality patient care for people living with diabetes.

This is an exciting opportunity for a professional to further advance outpatient skills and broaden his/her knowledge base about diabetes care and management.

**Position Qualifications**:

- Current Registration with the College of Nurses of Ontario
- Certification as a Registered Nurse in the Province of Ontario required
- Diabetes Canada - Diabetes Educator Certification preferred
- Minimum of two (2) years RN experience, preferably in a community setting
- 1-2 years’ experience in diabetes education and care preferred
- Knowledge of and sensitivity to the impact of social, economic, environmental, and cultural issues in diabetes self-management
- Demonstrated respect for diversity, equity and accessibility with the ability to work collaboratively and positively with others
- Able to work under pressure and multi-task multiple priorities
- Excellent communication skills (Written and verbal)
- Evidence of continuing education
- Current BCLS Heart and Stroke
- Excellent attendance record.
- Adherence to all health and safety policies, procedures and provisions and the recognition and reporting of unsafe hazards and work practices

**Principal Position Duties and Responsibilities**:

- Plans, develops and implements education sessions for individuals and groups using the evidenced based standards (Diabetes Canada)
- Ensures patient assessment, education and documentation are maintained according to clinic standards
- Utilizes clinical judgment and collaborates with members of the diabetes team, to assess current health status, interpret diagnostic test results, provide education and support to patients with diabetes

(Gestational, at risk for diabetes, Pre-diabetes, Type 1 and Type 2).
- Provides communication with the referring physician/primary care physician that may include current status, recommendations, and plan of care.
- Maintains professional competency through networking with peers and other health professionals, continuous self-study and participation in diabetes seminars, workshops, and conferences.
- Assists in the development and delivery of clinic programs including one-to-one counselling, group teaching, and other initiatives to support people with diabetes

**Hours of Work**:
Monday & Wednesday 830-430pm
