**About the Opportunity**:
Reporting to the Manager, Planning & Development, the Consultant (Primary Health Care), is responsible for working across the Primary Health Care system to advance the strategic and operational priorities of the Primary Health Care and Chronic Disease Management Network.

The Consultant (Primary Health Care) will work according to the mission, vision, values and policies of NS Health and the Primary Health Care and Chronic Disease Management Network to support primary health care and chronic disease management system design. The Consultant (Primary Health Care) will be responsible for working with the network team, patient partners, primary health care providers, physicians, and other team members as appropriate to design integrated patient centered care pathways for primary health care and chronic disease management. The Consultant may also be assigned other priorities as identified by the Network, in alignment with priority areas.

**About You**:

- Baccalaureate degree from a health discipline/health science field is required
- Master’s Degree in Business Administration, Health Services Administration or equivalent combination of education and experience considered an asset
- Minimum of 3-5 years working experience and responsibilities in a health-related environment
- Experience in a primary health care, and / or chronic disease management setting is preferred
- Experience in system design methodology and approaches considered an asset
- Experience in planning, coordinating, developing, delivering and evaluating primary health care and / or community-based initiatives
- Knowledge of the attributes of high-performing primary health care systems considered an asset
- Knowledge of chronic disease prevention, chronic disease management, and population health approaches considered an asset
- Project management skills considered an asset
- Proven ability to work with a variety of stakeholder groups
- Proven ability to develop high quality project materials
- Excellent communication skills
- Possession of a valid driver’s license

**Hours**:

- Long Assignment Full-Time Position; 75 hours bi-weekly
- Beginning March 1, 2023 with an expected end date of Aug 1, 2023
- Shifts may include evenings and weekends (rare)

**Compensation and Incentives**:
$35.27 - $44.09 hourly
