**Salary**

**$31.56 - 33.28** depending upon applicants' length of seniority (as per the Collective Agreement)

**Job Summary**

The Outreach Worker reports to the Designated Manager and works in accordance with the mission and philosophy of Lookout Housing and Health Society including following Lookout’s Code of Ethics. Outreach workers provide a variety of support, direction, assistance and advocacy services to clients including, but not limited to, those facing challenges with stable housing, addictions, mental health, HIV etc. The Outreach Worker provides support and advocacy for treatment and follow up services for those who do not adequately utilize needed services.

**Job Duties**
- Accepts guest referrals from the Ministry offices. 
- Encourages guest to use community resources. Develops and fosters a good working relationship between the organization and such resources. 
- Maintains regular ongoing supportive communication between the guest and their Employment and Assistance Worker (EAW). May provide life skills training with regarding to behaviour management and/or money management; and poverty rights access of entitlement. 
- Assists guests in conducting scheduled annual/financial reviews and employment plans. 
- Maintains ongoing contact with the various guest’s supervising Employment and Assistance Worker and the Manager of the local office(s) from which referrals are received, with regard to guests, and provides progress discussions and/or reports. 
- Maintains appropriate records and ensures that all documentation is accurate and complete; the monies and cheques administered are appropriately receipted; and all document transmissions between guests and Ministry offices are recorded and documented. 
- Ensure that all work areas are maintained in a clean and tidy manner and that required supplies are in stock. 
- Will discuss, isolate and assess issues and develop a flexible plan of action
- Will ensure basic needs of clients are met and network, as needed, with community services
- Will provide crisis intervention as necessary, administer finances and medications or arrange coverage
- Complete required records and maintain statistical data
- Provide assistance for new staff, volunteers or students
- Job duties are complex and may have a few disagreeable factors
- Responsible for complying with and contributing to all aspects of health and safety program

**Qualifications & Competence**
- Related Diploma or Certificate; or minimum Grade 12 education plus two (2) years of recent related experience; or equivalent combination of education, training and experience; and prefer a minimum of one (1) year case management experience
- Familiarity with community resources; particularly addiction, mental health, welfare, and harm reduction
- Must have current Occupational First Aid Level 1 Certificate and a Class 5 Driver’s Licence and Driver’s Abstract
- Must have two doses of COVID 19 Vaccine
- Crisis Intervention Skills Training is an asset
- Ability to work independently or in a team setting
- Strong ability to manage stress and organize workload
- Excellent communication, writing, documentation, and organizational skills
- Understands and maintains clientele/worker boundaries
- Ability to work with disadvantaged and challenging adults in a diverse environment
- Strong physical and mental ability to perform work tasks and operate job related equipment
- A minimum two (2) years of sobriety if you have had concerns related to alcohol and/or drug use
- Criminal Record Clearance - Vulnerable Sector
