**ADVANCED PRACTICE PHYSIO MOBILITY CLINIC** (JOB ID: 1248)**:
The Advanced Practice Physiotherapist promotes excellence and provides leadership in the core functions of; expert practice, teaching and learning, research and evaluation, professional leadership and consultation. In the area of expert practice, the Advanced Practice Physiotherapist provides assessment of patients attending outpatient Rheumatology and Orthopedic clinics and provides comprehensive health services with an emphasis on education, self-management, health promotion and disease prevention. The role encompasses triaging to other specialty services to maximize patient outcome and improve efficiencies. The Advanced Practice Physiotherapist is responsible for developing, coordinating and monitoring the interdisciplinary plan of care for these select patient populations. The Advanced Practice Physiotherapist integrates physiotherapy skills and knowledge within a broad scope of advanced clinical practice and assumes a leadership role in influencing physiotherapy practice/provision of care, education and research across the organization.
**DUTIES & RESPONSIBILITIES**:
- The Advances Practice Physiotherapist works as a member of a multidisciplinary team and in collaboration with physicians caring for patients seeking specialized rheumatology and Orthopedic services
- Works effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care- Contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community
- Identifies and addresses educational needs for patients and their families
- Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care
- Is committed to, and participates in, ongoing quality improvement in the organization
- Provides patient care in accordance with all professional and organizational standards**QUALIFICATIONS**:
- 5 years of clinical expertise in the care of arthritis patients;
- Must have a MSc degree in Physiotherapy (or equivalent)
- Must have successfully completed an approved program for advanced practice in Physiotherapy (E.g. Advanced Clinician Practitioner in Arthritis Care).
- Must be registered for independent practice and be in good standing with the College of Physiotherapists of Ontario;
- Academic appointment with an academic institution preferred.
- Current CPR certification
- Excellent communication, critical thinking and interpersonal skills
- Proven flexibility and adaptability in a fast-paced environment
- Excellent time management, organizational and prioritization skills as relates to patient care
- Demonstrated commitment to and involvement with own profession (e.g. membership with professional association, etc.) preferred
