**L’Annexe - Case Manager II & Group Facilitator**

**JOB SUMMARY**
L’Annexe, the Ometz Centre for Young Adults, is a diverse, LGBTQ+-affirming Jewish social service and community space open to everyone between the ages of 16 - 35. L’Annexe collaborates with young people as they build autonomy, engage in community, and develop their unique vision of well-being. As a member of an interdisciplinary team made up of youth advocates, social workers, creative arts therapists, and educators, the Young Adult Case Manager & Group
Facilitator provides both one-on-one support and group programming to enhance young adults’ social, emotional, physical, and financial well-being and autonomy.

**RESPONSIBILITIES, including but not limited to**:

- Develop and facilitate therapeutic, wellness, and social groups
- Develop and facilitate life skills workshops, community-building activities, and special events
- In collaboration with L’Annexe team, conduct ongoing evaluation of group programming, in response to evolving client needs
- Provide psycho-social support, including supportive counseling, coaching and accompaniment around life and social skills
- Support client in development and achievement of an individualized plan that includes concrete goals, actionable items, and timelines towards social, emotional, financial, and physical well-being
- Connect client to external resources, including public services, and collaborate with service providers to ensure clarity of roles and efficiency of services
- Fulfill all administrative and data requirements, including regular documentation of progress notes
- Represent L’Annexe within Agence Ometz and the broader community
- Serve on intra-agency and external committees
- Actively participate in clinical and peer supervision

**REQUIREMENTS**
- Bachelor’s degree in Social Work, graduate diploma in Youth Work, or professional equivalent
- Minimum of 2 years experience working with young adults (ages 16-35)
- Fluent bilingualism in both official languages; written and spoken. We require English communication skills due to the majority of our clientele being from the English speaking community.
- Commitment to providing LGBTQ+ affirming services
- Knowledge of Montreal and Quebec social service network and government programs and services
- Knowledge of Jewish community, a strong asset

**This position requires the selected applicant to undergo vulnerable sector police check.**
