The purpose of the position is to ensure all relevant information is received for all incoming requests for service that are received in the Central Intake Department. To provide timely feedback to referral sources where there is missing information. To utilize clinical judgment to prioritize and process requests in an effort to decrease wait times. The department is open 7 days a week from 0800-2300.

**KEY ACCOUNTABILITIES**:

- Receive all incoming referrals/requests for service.
- Review referral for completeness; contact referral sources for missing data elements.
- Provide written communication about the receipt of a referral, possible wait times, and next steps.
- Screen to ensure incoming referrals are appropriate; obtain all necessary information in order to properly assess referrals; provide recommendations regarding possible inpatient and outpatient registrations; assist referral sources to navigate Mental Health and Addiction services more appropriate for their needs; documents all outpatient contacts for statistical purposes as per hospital/program policy.
- Identifies level of priority for the request and next steps (i.e. request for more information; decline letter)
- Ensure client needs are met in the most efficient and effective manner, including managing the wait list of referrals, processing referrals in a timely manner and maintaining communication with referring source and program areas.
- Identifies key/essential data elements; identify high risk situations and provide early interventions
- Support/augment the role of the Clinical Access Coordinator
- Assist in the evaluation of key indicators

**REQUIREMENTS/QUALIFICATIONS**:

- Member in good standing of the College of Nurses of Ontario as a Registered Practical Nurse
- Minimum 3 years of recent and related experience in a mental health service/hospital setting preferred
- Demonstrated experience with navigation and referral services.
- Preference for formal training in patient navigation.
- Current experience with psychiatric diagnosis and disorders and theories of general and mental health theory
- Demonstrated knowledge of and experience with services within the NSMLHIN system for individuals with or without a mental illness
- Strong computer software skills
- Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
- Demonstrated reasoning/analytical, problem-solving, teamwork, interpersonal and organizational skills
- Proven written and verbal communication skills
- Proven ability to adapt readily to change
- Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation), and reflects an optimistic and positive attitude
- Preference for proficiency in French/English language skills
