**Position Overview**:
The Mental Health Case Manager position will be embedded in Child and Adolescent Mental Health Services, responsible for the review, intake, and acceptance or rerouting of all internal referrals to Child and Adolescent Mental Health Services.

This position supports a collaborative, multidisciplinary approach aimed to direct patients to the right care, at the right place, at the right time. Utilizing evidence based, standardized strategies and assessments, this Case Manager role promotes a transparent, equitable and accessible process for all patients, physicians, and clinicians.

This individual supports the patient journey through face-to-face and/or telephone interviews, while also gathering information necessary to direct the referral to the program which best meets the current needs of the patient.

**Position Summary**:

- Provide intake assessment and support patients referred, living in the catchment area.
- Screen, enroll and orient new patients to THPs Child and Adolescent Mental Health Services.
- Assist in care planning, transitional and discharge planning.
- Provide information and supportive services to other referring agents and liaise closely with other service providers to ensure optimum delivery of services.
- Matching patients to appropriate resources utilizing standardized assessments, service criteria, established standards, and clinical judgment.
- Follow-up with individuals and referrers as required to ensure timely and appropriate transition and system navigation.

**Qualifications**:

- Bachelors Degree in a health related discipline
- Registration in good standing with a regulatory college preferred
- Experience reviewing, triaging and directing referrals in a healthcare or community setting
- A second language (Cantonese, Farsi, Italian, Korean, Mandarin, Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred.
- Ability to multi-task, with excellent organizational skills.
- Understanding of the school system and support within the Mississauga community.
- Broad knowledge of THPs Mental Health Ambulatory Programs, as well, external programs and services within Mississauga.
- Excellent interpersonal skills, including problem solving and conflict resolutions skills.
- Strong written and verbal communication skills.
- Comprehensive knowledge of mental health, addictions, and trauma related conditions.
