Outreach Worker **POSITION**:

- EMPLOYMENT OPPORTUNITY IN WINNIPEG’S NORTHEND Hope Centre Health Care Inc is seeking a Community Outreach Worker person to join our team.
- _**JOB DESCRIPTION**_ 1) To provide advocacy for those requesting support in areas such as education, social assistance, child welfare, running support groups.
- 2) Provides a communication link between the client, the clients family members and/or care provider, and the appropriate HCHC department and other community agencies
- 4) Encourages, educates and supports clients in advocating on their behalf
- 5) To facilitate the communication between clients and other community agencies.
- 6) Conducts home visits to clients when necessary
- 7) Provides appropriate referral to clients with health and social issues
- 8) Provides community groups, facilitates projects that encourage the development of the community around identified issues of concern.
- 9) Develops and participated in group planning sessions that will benefit the community clients
- 10) Responsible for recording and keeping up to date records of all client inter action the in electronic Medical Records. (EMR),
- 11) Work in a multi
- disciplinary team of professionals.
