Models of Care Practice Coordinator

**Job Description**:
The **_Models of Care Practice Coordinator_** will provide support to front line clinical teams and leaders in the successful achievement of novel models of care with a focus on employee retention and improving the quality of the patient experience throughout the continuum of care. Reporting to the Manager, Professional Practice & Nurse Residency Program, this role is an integral member of the Professional Practice team who holds a pivotal role in championing employee well-being resulting in high quality patient care for patients and families. The role extends across the continuum of care within the inpatient/ED environments to best support patient care and sustainable services.
**Key Accountabilities**:
Provide Leadership to ensure processes are in place for achievement of safe, quality patient care through the models of care project including:
- Standards of Excellence
- Performance Indicators
- Policies and protocols
- Excellence and Innovation
**Quality and Safety**:
- Understands and implements delivery of care strategies with a focus on safety and quality care delivery.
- Supports and implements policies, procedures, guidelines & legislation,
- Champions and promotes implementation to facilitate changes identified by auditing, workflow mapping and engagement with key stakeholders.
- Contributes to the development of key performance indicators and monitoring
**Qualifications**:
**Education**:
- Baccalaureate preparation in a health-related field required; Masters prepared preferred.
- Current registration and a member in good standing with a regulatory college required
- Demonstrated ability to develop key partnerships with partners and other inter-disciplinary team members to promote a high-quality patient centred care required
- Current or previous Professional Practice or education related experience an asset.
- Project management & Change management experience preferred
**Experience**:
- Minimum of five years clinical practice experience with demonstrated clinical excellence in an acute care environment.
- Experience in a leadership role with the ability to work effectively within an interdisciplinary team.
- Demonstrated knowledge of planning, developing, implementing, and auditing clinical workflows, pathways, and policies.
- Working knowledge of Accreditation Canada Standards and ROPs.
- Working knowledge and understanding of College of Nurses Standards as well as scope of practice for interprofessionals practicing within the clinical environment
- Demonstrated understanding of the RHPA legislation
- Demonstrated leadership ability.
- Demonstrated skills in coaching, mentoring, facilitation, and team building.
- Proven ability to achieve objectives within set time frames
