The Caseworker maintains a caseload and provides clinical leadership and resources to the assigned unit. The Caseworker provides clinical assessments, counselling, treatment plans, advocacy, team training, case management, group work, and family work. The Caseworker also provides clinical consultation to group Living and educational/vocational teams. The Caseworker works with multiple teams and is a key communication link between stakeholders through written reports, consults, and daily communication.

**Education and Professional Certification**

***Experience**
- Minimum one (1) year of experience working with individuals with complex needs, behavioural issues, developmental disabilities, mental health disorders, and/or other related issues.
- Experience with clinical interventions and assessments for vulnerable persons.

**Duties and Responsibilities**

**Services**
- Develop individual and family treatment plans for participants in collaboration with internal and external stakeholders.
- Provide clinical oversight and lead the implementation of individual and family treatment plans including all aspects of participation in the treatment program, completion of documentation, and providing information and feedback to the stakeholders.
- Conduct clinical assessments and refer for specialized services (psychological assessments, counselling, clinical resource needs, adjunct therapies, and psychiatric services) when or if deemed necessary. Communicate with referral agencies and attend clinical appointments with participants when required.
- Conduct individual, group, and family counselling.
- Advocate on behalf of the participants.
- Lead planning conferences, family consultations, and conferences with health care professionals. Participate in educational conferences and conferences with other stakeholders as required.
- Communicate and collaborate with internal and external stakeholders including families and referring agencies.
- Act as a liaison between the group living and education programs regarding the participants service plan.
- Provide oversight and influence related to the therapeutic milieu of the treatment program, providing employee training or education sessions when required.
- Provide post-crisis support to the participant and direct clinical follow-up as appropriate.
- Complete reports and document all casework in a clear, accurate, and timely manner. Disseminate important information to families and referring agencies.
- Perform other related duties as assigned.

***Leadership**
- Set the direction for the treatment program by integrating quality improvement initiatives and encouraging reflective practice.
- Share specialized knowledge with the treatment team and provide professional consultation and training to assist with the professional development of the team members.
- Provide support and clinical consultation services as required to the multi-disciplinary team.

**Management of Resources**
- Manage and evaluate specialized or adjunct therapies and clinical resources within an allocated budget.
- Coordinate and collaborate with community resources when required.
- Influence the operational budget planning as it relates to treatment plans and other services.
- Provide input into budgets for special needs, events, or education sessions.

**Safety**
- Maintain, promote, and reinforce safe work habits, practices, and procedures.
- Adhere to written or verbal agency and government regulations, policies, practices, and instructions.

**Additional Requirements**
- The incumbent is responsible for completing all mandatory training requirements for the position as per agency policy and guidelines.
