Reporting to the Director of Admissions, the Intake Coordinator conducts intake processes to ensure that prospective clients are provided with timely, appropriate information and that the residents who are admitted to GreeneStone are a fit for the facility. The intake coordinator works together with the various departments of the facility to ensure a smooth admission process.

The incumbent is responsible for completing all the documentation, assessment tools, financial, and transportation needs, and any exceptions for those individuals that are admitted to the program. The intake coordinator ensures that all of the intake calls are responded to in a professional and timely manner and also maintains the information in the FYI system.

The position is also responsible for reporting statistics and managing information regarding intakes throughout the calendar year. As the first point of contact for the client or their family, the intake coordinator needs to understand their needs and communicate effectively while providing an exceptional customer experience.

**Responsibilities**:

- Completes intake questionnaires in FYI with prospective clients or their families. Responsible for using sound judgment in completing the necessary fit requirements (intake on FYI, assist, QOL measure) to ensure a proper fit to GreeneStone. Maintains records on FYI.
- Confirms that all admissions records are accurate, in place, and maintained.
- Maintains client records (confirmation/payment/extensions) in a confidential manner (hard copy and soft copy on FYI).
- Communicates with residents and family members in an effective and professional manner maintaining boundaries.
- Coordinates the admission dates to ensure space is available and admissions are appropriately timed.
- Manages the extension for all residents wishing to stay longer when the clinical team deems it clinically appropriate.
- Ensures that the financial and transportation arrangements for each resident are in place for each admission.
- Ensures that all inquiries regarding residents that have been admitted are directed appropriately to the clinical team in the facility.
- Prepares discharge summaries, uploads to a shared drive, FYI and forwards to caseworkers as appropriate.
- Prepares invoices for payment as required.
- Participates in providing education sessions.

**Qualifications**:

- Post-secondary degree in social work, psychology or addictions along with a minimum of 3 years’ experience
- Ability to multitask and set priorities based on client needs.
- Must have the ability to be professional, objective and empathetic while interacting with the residents who are seeking treatment.
- Must demonstrate ethical behavior and sound judgment that supports and aligns to the GreeneStone standards of care, vision and values.
- Computer skills, Intermediate experience with Microsoft Products and Outlook, experience with or willingness to learn to use an electronic medical record (FYI DB). Must be able to document insights and observations in clinical records in a concise and articulate manner.
- Must possess a high degree of professionalism and be able to keep information confidential and treat sensitive clinical information with the upmost discretion and respect.
