**GENERAL DESCRIPTION**
The Chaplain is responsible for the provision and coordination of spiritual and religious care to the residents and their families and to provide Quality of Life Services for the residents within Glen Hill Terrace and the Durham Christian Homes (DCH) Family of Homes.
**RESPONSIBILITIES**
**Christian Community**
- Reflect the DCH Mission statement and reinforce the Christian community at Glen Hill Terrace.
- Support group faith services within Glen Hill Terrace.
- Maintain working relationship with member of community faith groups and volunteers.
- Encourage faith expression among residents.
- Promote outreach in the community and beyond.
- Provide memorial services quarterly to celebrate the lives of those who have passed.
- Responds to and assists with enquiries regarding Spiritual Care at Glen Hill Terrace.
- Provide hospital and in-house visitation as needed for all residents.
- Lead or conduct funeral services as requested and able.
- Provide end of life and palliative care supports.
- Coordinate spiritual care supports to residents.
- Coordinates and oversees spiritual care volunteers.
**Resident-Focused**
- Continually improve the quality of service and care that we provide.
- Prioritize daily activities to reflect a resident first approach.
- Deliver spiritual care services to residents on a day to day basis.
- Supports visiting clergy and volunteers in providing regular ongoing ministry to resident of a wide variety of faiths.
- Delivers services on the weekend when required.
- Supports residents and family as required.
**Workplace excellence**
- Support departmental and whole home in services to promote a safe workplace and provide opportunities for learning and advancement.
- Responsible for working safely in compliance with accepted safe work practices, procedures and legislated health and safety standards. Employees must: use and wear the equipment, protective devices and personal protective equipment as required; report hazard, accident, and the absence of or defect in any equipment or protective devise; attend all required health and safety training, and follow all established safe work procedures.
- Participates in home committees as appropriate and/or as required.
- Participates in Interdisciplinary Team Conferences as required.
- A standing member of the Palliative Care committee.
- Consults with interdisciplinary team concerning the spiritual needs of residents.
- Completes spiritual assessments as required.
- Document support provided as required.
**Fiscally Responsible**
- Adhere to departmental budgetary guidelines.
- Seek most financially economic and effective options.
- Use internal resources before external resources whenever possible.
- Meet deadlines in a timely manner to avoid extra costs.
- Utilize resources efficiently.
**Successful Continuum**
- Promote the benefits of DCH continuum internally and externally.
- Provide consistent level of service excellence in the continuum.
- Identify changes in resident condition or needs in order for Glen Hill Terrace to respond with the appropriate care of service.
- Communicate concerns or issues regarding the resident’s changing needs to the appropriate departments.
- Support residents and family going through the continuum (counselling, encouragement, comfort).
**Innovative Approaches**
- Continually investigates and provides ideas, resources, suggestions and research supporting the move to new and improved methods of delivering spiritual and religious services.
- Participates in continuous quality improvement by providing monthly statistics and submitting them to the Administrator and/or C.E.O.
- Completes Quality Monitoring and Analysis reports as necessary and provides insight into data.
- Performs audits and collect, inputs and supports data as necessary.
- Documents care and services provided within the nursing software.
**Qualifications**
- One year experience in a long term care home preferred.
- Authorized pastoral care clinician.
- Post graduate education and training in multi-faith programing.
- Graduate from a theological college at the post graduate level.
- Flexibility with working hours in order to support residents.
- Experience with palliative and end of life care and supports.
**Employment will be conditional on providing a current police check, including vulnerable sector screening; as well as providing acceptable proof of being fully vaccinated against COVID-19, including the 3rd booster.**
