The Integrated Community Access Program (ICAP) serves as a central point of access for most inpatient and outpatient services at Ontario Shores Centre for Mental Health Sciences (Ontario Shores). Specialized recovery focused staff provide a range of triage, intake, system navigation and outpatient follow up. ICAP offers a range of expert services that include assessments, consultations, behavioral support, shared care and follow up that can be provided in a traditional office-based setting, in the community or via Telemedicine based on the unique needs of each individual.

**Position Description**:

- Mental Health system navigation
- Patient portal
- Identifying need for, and completion of assessments
- Linkage with community providers, and act as a liaison with community and primary care providers
- Psychosocial education, develop treatment / recovery goals and support the development of crisis plans

**Requirements**:

- Registration as a Healthcare Provider, Undergraduate Degree, or possession of a current certificate of competence as Registered Practical Nurse if applicable.
- Current CPR-HCP is required.
- Demonstrated knowledge and caseworker experience with Mental Health and addiction community resources is an asset.
- Ability to make effective referrals, reflecting in depth knowledge of community resources and strong collaborative skills in working with community partners and health care providers.
- 3-5 years psychiatric experience in a mental health facility or service.
- Demonstrated knowledge of the College Standards of Practice and commitment to maintaining reflective practice standards as described by the College (if applicable).
- Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
- Ability to work effectively individually as well as part of an inter professional team.
- Demonstrated knowledge of psychotropic medication.
- Proven ability to exercise mature, effective judgment and behavior in emergency situations.
- The ability to work with clients of diverse backgrounds and proficiency in a second language would be considered an asset.
- Current driver’s license;
- Ability to perform the physical demands of the position
- Variable hours may be required, due to patient demands
