Community Care is currently seeking a Health promoter to develop, implement and evaluate accessible and sustainable health promotion programs and community initiatives to address individual, family and Community needs.

**KEY ACCOUNTABILITIES**
- Support the agency in meeting service goals, by preforming community needs assessments and coordinate, develop, implement, monitor and evaluate programs to respond to those needs.
- Educate and advocate on behalf of the Community and our clients.
- Identify and manage health promotions resources, and provide leadership in agency health promotion activities.
- Provide referrals and collaboration to ensure ongoing care of clients.
- Establish and maintain links and develop partnerships within Community care and other agencies in the community.
- Ensure the organizational vision and values are incorporated in all processes.
- Ensure support of Community Care strategies and adherence with legislation.
- Supports a continuous quality improvement culture throughout Community care.

**KEY COMPETENCIES REQUIRED FOR THIS POSITION**
- Degree in health, social science or related discipline, Masters preferred.
- 3 to 5 years’ experience in Health promotions or Community development position working in a culturally diverse community based settings.
- Demonstrated knowledge of Community resources, health promotion and Social determinates of health and public health policies and their impact on marginalized communities.
- Experience in developing and managing Community based projects including financial management reporting and budgetary preparation.
- Experience in conducting/participating in research on health needs through community participation.
- Excellent oral and written communication skills
- Work independently as part of a multi
- disciplinary team.
