QUALIFICATIONS:

- Bachelor's degree in Social Work.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Two years in the past five years of working experience in an inpatient or outpatient medical / physical rehabilitation setting.
- Must possess therapeutic skills to conduct psycho-social interventions, crisis intervention and brief therapy interventions.
- Knowledge of and experience in case management.
- Skilled in counseling and educating individuals, families and groups regarding coping strategies and cognitive behavioural change theory related to lifestyle changes.
- Knowledge of psychosocial and physiological impact of chronic illnesses and conditions, heart and stroke disease an asset.
- Understanding and knowledge of available community resources.
- Knowledge of all relevant legislation (i.e. Health Care Consent Act, Substitute Decision Act, Mental Health Act, Public Guardian and Trustee, Child Protection Act).
- Demonstrated professionalism and community relations skills while dealing with the public (patients and families), as well as various medical and non-medical professionals.
- Evidence of effective customer service, organizational, interpersonal, communication, judgment, critical thinking and decision-making skills.
- Ability to work effectively within a multi-disciplinary team.
- Flexible, self-directed and able to work effectively with mínimal supervision.
- Consistent and regular attendance required.
- Proficiency in both official languages (English & French) will be considered an asset
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code
