Harm Reduction Worker

**Job Summary**:
We are seeking a dedicated and compassionate Harm Reduction Worker to join our interprofessional team. As a Harm Reduction Worker, you will provide support and social and health care navigation to an underserved community with various social, economic and health-related barriers. An exciting opportunity, this position is an integral role in the operation of our Mobile Health Unit.
The Mobile Health Unit is a project operated by Recovery Care in collaboration with Eastern Ontario Health Unit and Cornwall Community Hospital. The Mobile Health unit provides substance use health assessments, care navigation services as well as infectious disease screening and management. A clinic on wheels, the Mobile Health Unit attends weekly locations that are otherwise underserviced, such as drop-in centers, shelters and communities in the catchment of Cornwall.
**Duties**:
- Engage with clients to build a supportive relationship.
- Support clients by developing and providing resources related to substance use health.
- Develop superior knowledge of community support services and resources.
- Process paperwork related to this role for patients, requesting/requiring transfer to alternate treatment facilities and maintain exceptional documents.
- Collaborate with other healthcare professionals and community partner organizations to coordinate care for clients.
- Educate clients and their families about the nature of substance use, recovery strategies, and harm reduction measures.
- Conduct interviews with clients to obtain substance use history and mental health needs to assist in care navigation.
- Provide education on treatments offered with Recovery Care.
- Support the development of social and harm reduction programming in collaboration with the multi-disciplinary team and management.
**Qualifications & Skills**:
- Valid G Class Ontario Driver's License.
- Knowledge of medical terminology related to addiction treatment
- Excellent interpersonal skills for building rapport with clients and establishing a supportive relationship
- Compassionate and non-judgmental approach towards individuals struggling with addiction.
- Excellent communication, organization and time management.
- Knowledge and comfort working with individuals in rural and remote communities, as well as the barriers related to accessing care for such populations. Comfort in working with individuals that are unhoused, shelter-based or under housed.
- Fluency in English is required. Fluency in French is an asset but not required.
- Experience in social service navigation.

**Salary**: $23.00-$28.70 per hour
Expected hours: 16 - 24 per week
