Addiction Intensive Case Manager

Four Counties Addiction Services Team (Fourcast) is a community-based addiction treatment service provider that strives to provide the highest quality of evidence-based addiction treatment and case management services. Fourcast offers services across 5 sites in the region. The community treatment program staff work in-office and community settings and are often co-located and integrated with other organizations to reduce barriers for those accessing services.
**About The Position...**
**We are looking for an experienced individual for this position working Monday to Friday 8:30am - 4:30pm.**
The Hospital to Homes program provides short term case management, navigation and counselling to support patients transitioning from hospital to community. This position acts as an integrated member of the Hospital to Home programs at Peterborough Regional Health Centre, Northumberland Hill Hospital and Ross Memorial Hospital.
The role requires the Addiction Intensive Case Manager (Hospital to Homes) to be an advocate and educator within the hospital teams. The Addiction ICM supports comprehensive coordinated care and service planning for identified patients and builds supportive relationships to improve access and plan a supportive transition to community health and social services.
**What We're Looking For**:
**Education**
- Bachelor of Social Work, Psychology, or related degree, with a postgraduate certificate in Addictions and Mental Health, or an equivalent combination of work experience and education.
**Experience**
- Minimum 3-5 years’ experience working in a related position preferred, ideally in a community-based setting and/or hospital setting.
- Advanced skills in addictions interventions including counselling/therapy, crisis intervention, advocacy, mediation, and conflict resolution.
- Demonstrated experience working with people with serious mental illness and complex personal needs.
- Experience using evidence-based screening and assessment tools.
**Certifications, Licenses and Records**
- Valid Ontario Class G driver’s license.
- Valid First Aid/CPR.
- Non-Violent Crisis Intervention (NVCI) certificate.
- Certification in GAIN assessment would be an asset.
- Applied Suicide Intervention Skills Training (ASIST) an asset.
- Satisfactory Vulnerable Sector Check is required.
