The Registered Practical Nurse with the Behavioural Supports Ontario (BSO) LTC Lead Team is part of a team that provides education and support to older adults who are demonstrating responsive behaviours that may be associated with various types of dementia, delirium, mental health, addictions or other neurological conditions and/or are transitioning to or from a long-term care home (LTCH), retirement home or community within the geographical boundaries of Lambton County.

**Reporting to the **Manager of Integrated Services, **the incumbent will be responsible for the following functions**:

- conduct necessary assessments with referred clients;
- support client, BSO Lead Team and LTCH staff in the development and implementation of a Behavioural Support Plan tailored to the client’s needs and issues designed to improve quality of life and assist with the support of responsive behaviours;
- regularly assess, evaluate and make changes based on new information gathered through observation, additional assessment and feedback from colleagues and caregivers;
- Consult with RN manager, GMHOT LTC and/or BSO core team for complex cases
- provide timely and accurate record keeping and reports as required;
- provide support, mentorship and consultation to colleagues, family members and/or other care-givers to assist in support of escalating responsive behaviours;
- maintain strict confidentiality and follow all other required policies and procedures;
- consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on CMHA and Behavioural Supports Ontario;
- effectively participate as a member of the BSO LTC Lead Team; attend staff/team meetings, client care conferences, other multi-disciplinary meetings, committee meetings etc. as required;

**Qualifications**:

- Registered Practical Nurse Diploma from a recognized post-secondary institution and current registration with the College of Nurses;
- Minimum two to three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions;
- a valid Ontario Driver’s Licence and access to a vehicle;
- demonstrated knowledge and best practice in dementia, delirium, mental health issues and their effect on persons, families and caregivers;

**Skills required**:

- must have a broad knowledge of bio-psychosocial rehabilitation and the therapeutic relationship;
- Training/knowledge of P.I.E.C.E.S., U-First, and GPA approaches
- strong assessment, intervention and Behavioural Support Planning skills;
- crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills:

- crisis or suicide intervention training is an asset;
- training in Non-Violent Crisis Prevention is an asset;
- CPR training;
- well-developed problem solving abilities and interpersonal skills;
- ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
- sound understanding and commitment to the principles of Behavioural Supports Ontario (BSO)

**Hours of Work**: 35 hours per week. Flexible hours including days, evening and weekends are required to meet the position requirements.
