**Job Summary**

**Duties**
- Conduct assessments to evaluate clients' needs and strengths.
- Develop, implement, and monitor individualized treatment plans.
- Provide counseling and support to clients dealing with emotional, psychological, or social issues.
- Advocate for clients by connecting them with community resources and services.
- Maintain accurate records of client interactions and progress.
- Collaborate with multidisciplinary teams to coordinate care and services.
- Educate clients and families about available resources and coping strategies.
- Participate in ongoing professional development and training.

**Requirements**:

- Bachelor’s degree in Social Work (BSW) or a related field; Master’s degree in Social Work (MSW) preferred.
- Valid state licensure as a Social Worker is required.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a team.
- Knowledge of community resources and social service programs is a plus.
- Experience in case management or direct client service is preferred.
- Proficiency in using electronic health record systems is desirable.
