**Job Summary**
The Minds in Motion® Coordinator is responsible for partnership engagement and management for the delivery of
Minds in Motion in the communities we serve. The Minds in Motion® Coordinator is responsible for the preparation and delivery of the weekly program which includes the planning and the facilitation of the Physical Fitness and Social
Recreational component of the program.
The Minds in Motion® Coordinator is also responsible, for the coordination of volunteers, for screening, training and supervising volunteers who are supporting program delivery.
**About You**
**Education and Formal Training Requirements**
Diploma or degree in health care, community recreation or therapeutic recreation studies, social work, gerontology, or other related discipline
Experience working directly with people living with dementia and their care partners.
A minimum of two years facilitating groups comprised of diverse individuals with complex needs, preferably in social and or therapeutic recreational setting.
Valid First Aid and CPR certification required.
Participation in the training provided by the Alzheimer Society, specific to Minds in Motion®, including: - Dementia
Inclusive Choices for Exercise - DICE - Minds in Motion program and evaluation overview - CCAA Fitness Training
- Dementia 101 and 102
History of working and supporting volunteers and/or student placements within a program environment
Willingness to participate in professional development and work actively with team members.
Ability to travel within our Tri-County - Lanark Leeds Grenville on a continual basis.

**Functions and Responsibilities**
Development and management of partnerships with community centres in which Minds in Motion® will be delivered.
Facilitation of social networking and relationship building among participants
Attendance at each weekly program session
Responsible for set up and clean up of program(s), facilitate and participate in program, encourage, and assist participants as needed.
Building strong internal and external relationships with clients, staff, volunteers, donors, and community stakeholders
Maintaining confidential, accurate client documentation records (Alayacare) and assessment of client progress.
