**Job Purpose**

The Primary care social worker provides social work services to the primary health care team and occasionally to the MOSH team. Reporting to the Social Work Team Lead and the Director of Community Mental Health, the primary care social worker is responsible for providing case management, service navigation and supportive counselling services in the primary care clinic and various community settings, working within an interdisciplinary team.

Working from an anti-black racist, anti-oppressive, harm-reductive and trauma informed lens, the primary care social worker must have an understanding to the needs of the North End community including those who experience homelessness, poverty, active addiction, and marginalization based on race, ethnicity, sexual orientation and gender identity. The Primary care social worker must have skills and knowledge of system and service navigation, skills in strength based intervention, knowledge of provincial and federal policies related to income, housing, pharmacare, and other benefits.

Education and Experience:

- Bachelor of Social Work (BSW) from an accredited University, is required.
- Licensed and in good standing with Nova Scotia College of Social Workers (NSCSW), required.
- Experience in health care social work, preferably primary care, preferred.
- Strong understanding and utilization of harm reductive principles, required.
- Knowledge and experience of working with people who have complex needs and who are experiencing substance use, homelessness, and/or poverty, preferred.
- Knowledge and experience working with historically marginalized populations, including African Nova Scotian, Indigenous, and 2SLGBTQIA+ communities, and providing culturally appropriate care and referrals, preferred.

**Position Responsibilities**

- Utilize a collaborative care approach to practice, working as a team member within the NECHC Primary Care Clinic team to provide coordinated care.
