Reporting to the Manager of Mirrors St, the Mirrors Housing Support Worker for Individuals is a vital part of the team, providing individualized, person-centred support to CLBC-designated Individuals participating in the Mirrors program. They will support CLBC-eligible Individuals to schedule and attend appointments, keep their suites clean, and access other resources. The Housing Support Worker for Individuals supports the development of daily life and living skills by working closely with CLBC Individuals.

**PRIMARY DUTIES AND RESPONSIBILITIES**:

- Provide individualized, person-centred support to CLBC eligible Individuals, including:

- Prepare, document, implement, monitor and evaluate support plans with each eligible participant’s integrated community teams
- Utilize harm reduction and strength-based interventions and approaches to support Individuals with goal planning, safeguarding and implementing behavioural support and safety planning.
- Identify potential problems; troubleshoot with eligible Individuals and report any difficulties to Mirrors Housing Support Staff and community-integrated teams
- Collaborate with management, housing workers and other professionals regarding CLBC-eligible participant needs and support
- Monitor participant’s mental health and substance use and determine when to provide additional support
- Recognize, analyze and respond to potential emergencies in and around the building
- Encourage and facilitate participation in decision-making regarding housing, programming and supports
- Create and maintain professional relationships with CLBC eligible Individuals, the community, and all stakeholders
- Follow CLBC-mandated principles of care to increase independence, autonomy and inclusion
- Liaise and actively plan with community resources including but not limited to; CLBC, Foundry workers, Integrated Case Management Teams, Umbrella HOPPS, health and health care practitioners, police and emergency services
- Be familiar with, build and maintain positive relationships with appropriate community resources
- Plan for and provide appropriate support for selected CLBC-eligible Individuals
- Track participant goals and outcomes using the CLBC-required matrix
- Participate in the reporting process by tracking statistics and outcomes of individuals and the program for the manager and CLBC reporting
- Support CLBC eligible Individuals in attending appointments, including but not limited to medical, psychiatric, and counselling to encourage and build life skills
- Coordinate referrals to essential services and programs for young adults
- Communicate with the rest of the team to provide support and coverage when required
- Transport clients to and from appointments
- Remains a part of the larger framework of Mirrors Housing Staff
- Perform other relevant job duties as required

**EDUCATION, EXPERIENCE AND REQUIREMENTS**
- Minimum 3 years experience working with homeless and marginalized groups of individuals in a direct support role
- Minimum 2 years experience with case management and individualized care
- Experience working with people with complex needs including developmental disabilities, FASD, ADHD, Autism and concurrent disorders
- Level 1 Occupational First Aid
- Food Safe Certificate
- Naloxone training
- Valid Class 5 driver’s license
- Access to reliable transportation
- Comfortable working in a pet-friendly environment.

**KNOWLEDGE, SKILLS AND ABILITIES**:

- Excellent interpersonal, verbal, and written communication skills
- Skilled in the use of MS 365(Teams, Outlook, Word, etc.)
- Ability to problem-solve and work both independently and as part of a team
- Ability to maintain strong judgment and professional attitude in demanding conditions
- Physical ability to lift 30 lbs, navigate stairs, and be on feet for extended periods
- Ability to work in environments with substances and substance smoke
- Knowledge of addiction, mental health, homelessness, poverty, and child and youth development
- Familiarity with psychosocial rehabilitation, trauma-informed care, harm reduction, and client-centered practice
- Understanding of mental health needs and addictions, with knowledge of community resources
