Job Summary:
**Duties**:

- Conduct assessments to determine the needs and goals of individuals requiring intervention services. by offering friendly visits and callers to isolated seniors.
- Ensure 50+ communities of Lachine have access to food by coordinating and implementing shopping food service.
- Facilitate social integration of isolated older adults
- Supervision and integrated volunteers within the services
- Intervene with complex cases and crisis intervention.
- Develop and implement intervention plans based on individual needs and the agency's mission.
- Provide one-on-one support and guidance to individuals during the interventions
- Monitor progress and make adjustments to intervention strategies as needed
- Collect data on individual outcomes and maintain accurate records
- Collaborate with other professionals and agencies involved in the intervention process
- Recruit and engage participants in the intervention programs
- Provide administrative support, including scheduling appointments and maintaining documentation

**Requirements**:

- Diploma in Special Care Counseling, Social Services or related studies or equivalent work-related experience in the community.

Experience in group dynamics and interpersonal relationships

Knowledge of and/or interest in community resources, particularly in older adults

Able to intervene in a complex situation

Good problem-solving skills and ability to manage stressful situations.

Bilingualism essential

Curious to learn new skills, flexible and adaptable

An understanding of seniors who have mental health illnesses, cognitive decline, and reduced mobility needs.
- Strong public speaking skills to effectively communicate with individuals and groups
- Demonstrated leadership abilities to supervise volunteers and guide participants through the intervention process
- Excellent organizational skills to manage multiple tasks and prioritize responsibilities
- Ability to collect and analyze data to evaluate the effectiveness of intervention strategies
- Experience in recruiting participants for programs or initiatives

**Salary**: $22.00-$24.00 per hour

Expected hours: No more than 35 per week
