**About the Opportunity**:
Reporting to the health services manager, Geriatric Medicine, the Dartmouth Community Geriatric Navigator, through an intake process, identifies clients/family needs, develops recommendations, and evaluate the most appropriate services and programs that will assist in meeting the frail elderly/complex case individual/family's goal in living in their own community for as long as possible. Works collaboratively with the client’s family physicians, Continuing Care Coordinator, diverse Nova Scotia Health Authority Health Care team and community supports/agencies. Supports access to the services and programs that enables successful pain for the client/family. The ultimate goal is to enable the client to live in their own community and have issues addressed before they encounter a crisis point and require a visit to emergency and/or hospital admission. The navigator acts as a resource for obtaining answers to questions and concerns.

**About You**:
We would love to hear from you if you have the following:

- Regulated health care profession is required (e.g. Nursing, Occupational Therapy, Physiotherapy, Social Work)
- Maintains current license/registration with the appropriate provincial and/or national regulatory boards
- Three (3) years relevant experience in a health care setting
- Knowledge and experience with community practice and frail elderly population
- Knowledge of Nova Scotia Health services/programs and Dartmouth services/programs, specifically in the Dartmouth area and health care facilities.
- Demonstrated leadership skills, critical thinking, verbal and written communication and interpersonal effectiveness
- Ability to nurture and maintain credible working relationships with a wide variety of stakeholders
- Must be able to work independently and as part of a team
- Competency in computer based programs (Microsoft office suites)
- Access to reliable transportation and travel within the zone for home visits is required
- Competencies in other languages an asset, French preferred

- Short assignment, Part-time / 0.65 FTE / 48.75 Hours Bi-weekly

**Compensation and Incentives**:
$33.97 - $42.89 Hourly
