**JOB** SUMMARY**: The Gahwajiyageho? Sehswa? ne:t (You Will Support the Families) reports to and works under the direction and supervision of the Program Supervisor. The Gahwajiyageho? Sehswa? ne:t (You Will Support the Families) will assist the Health Services Department and the Home & Community Care Program in the performance of statutory, operational and advisory services

The Gahwajiyageho? Sehswa? ne:t (You Will Support the Families) will provide support, education and resources to families and caregivers with the focus on supporting individual to live at home as long as possible.

The Gahwajiyageho? Sehswa? ne:t (You Will Support the Families) works within the policies and procedures established by Six Nations of the Grand River.

**BASIC QUALIFICATIONS**:

- Bachelor Degree in Social Work or related field OR

Social Service Worker or Practical Nursing Diploma
- Related experience working with and supporting caregivers
- Experience developing programs and facilitating groups
- Demonstrated ability to manage crisis situations
- Knowledge of the Haudenosaunee community and willingness and interest to work from a place of culture as foundation
